Phosphoglyceride crystal deposition disease mimicking a malignant tumor

Abstract: A 45-year-old woman who had undergone surgical repair for an atrial septal defect at the age of 7 years presented with multiple anterior mediastinal masses. Positron-emission tomography (PET) with 2-[18F]fluoro-2-deoxy-d-glucose (FDG) revealed high uptake in the masses. The findings were interpreted as mediastinal malignant tumors with dissemination. Biopsy was performed, and the histological diagnosis was phosphoglyceride crystal deposition disease without neoplastic changes. Although phosphoglyceride crystal… Show more

“…These PG crystals are characterized by numerous refractions under polarized light and positive staining with GHA 1 . PGDD results in the formation of single or multiple masses in the soft tissue or extra-articular bones, and these masses are often radiologically misdiagnosed as malignant tumors 9 . The present case is the first reported case of PGDD treated with curettage of the whole lesion and artificial bone grafting.…”

“…The FDG uptake in two previous reported cases of PCDD ranged from 13.6 to 26.0[ 3 , 4 ]. Our patient had even higher uptake, ranging from 34 to 37, in all of her many masses.…”

“…In previous reports, phosphoglyceride has been identified by the gold hydroxamic acid method, X-ray microanalysis, and microstamping mass spectrometry[ 1 ]. Clinically, on 18 F-fluorodeoxyglucose positron emission tomography ( 18 F-FDG-PET), phosphoglyceride deposits have been reported to show a maximum standardized uptake value (SUVmax) ranging from 13.6 to 26.0[ 3 , 4 ]. Here, we report a case of PCDD showing extremely high uptake on 18 F-FDG-PET in which Raman spectroscopy led to a definitive diagnosis of PCCD.…”

“…PCDD is extremely rare; to the best of our knowledge, only 12 cases, all from Japan, have been reported to date 1-10 . In 10 of these cases, the lesions developed at the site of postoperative scar or repeated injections on the body surface (abdominal wall 1 , 3 , 9 , buttock 2 and upper arm 3 ) as well as in internal organs (peritoneum 4 , spine 5 , anterior mediastinum 6 , 8 , cardiac wall 10 , and pelvic wall 7 ) a long time (20-45 years) after operation/injection. These findings suggest that tissue injury may play an important role in PG deposition, while its developmental process is still largely unknown.…”
