Hepatitis B virus (HBV) infection represents the most common cause of chronic liver diseases worldwide. Consequently, to the introduction of the universal HBV vaccination program, the prevalence of hepatitis B surface antigen was markedly reduced and less than 1% of the population of Western Europe and North America is chronically infected. To date, despite great advances in therapeutics, HBV chronic infection is considered an incurable disease. Ten hepatitis B virus genotypes (A-J) and several subgenotypes have been identified so far, based on intergroup divergences of 8% and 4%, respectively, in the complete viral genome. HBV-D genotype has been found throughout the world, with highest prevalence in the Mediterranean area. In the present review, several articles concerning HBV epidemiology, and phylogeny in Italy have been analyzed, mainly focusing on the changes occurred in the last decade.

The urgency of the problem of hepatitis B is due to the significant economic damage, medical, social and epidemiological significance of this disease, the ubiquity, severity, high frequency of adverse outcomes of infection, active involvement in the epidemic process of persons of reproductive and working age. The socio-economic significance of hepatitis B is evidenced by the morbidity, prevalence and mortality from this infection. The decrease in the incidence of acute hepatitis B, which is observed in Russia at the present time, is not a reason for an optimistic view of the problem, since the incidence of chronic hepatitis B over the past five years is almost at the same level (9–10 cases per 100 000 population). To date, laboratory diagnosis of viral hepatitis is based on the determination of specific markers by enzyme immunoassay. The use of molecular genetic methods opens up new prospects in the study of infection, allows us to identify patterns of manifestation, mechanisms of regulation of the epidemic process, including the study of the variability of the pathogen in space and time. The use of molecular genetic research methods should be considered not as an alternative, but as a mandatory addition to the regulated diagnostic schemes, which will effectively identify the pathogens of viral hepatitis, assess the identity of viral isolates and on this basis improve epidemiological surveillance and control of these infections.

Резюме. Вирусный гепатит В является серьезной проблемой здравоохранения всего мира, России и ее регионов. Причиной этого является его широкая распространенность в разных этнических и географических группах, разнообразие проявления клинических форм, а также исходов. По данным ВОЗ, около 50 млн человек в мире ежегодно заражаются вирусом гепатита В. Развитие молекулярно-генетических методов исследования позволило расширить представления о биологии возбудителя. Генетическая вариабельность генома вируса гепатита В привела к появлению 10 различных географически распространенных генотипов, которые обозначены буквами от А до J. Внутри ряда генотипов выявлены субгенотипы, отличающиеся между собой на 4-8% полных геномных последовательностей. В работе дана таблица географической распространенности различных генотипов вируса гепатита В в мире. Географическое распределение генотипов вируса гепатита В тесно связано с эндемичными регионами и коренным населением, проживающим в них. Так генотипы В и С связаны с населением азиатских стран, генотипы А и D распространены среди европейских стран и в США. Генотип D является преобладающим типом среди пациентов с гепатитом В в различных регионах Турции. Филогенетический анализ нуклеотидных последовательностей изолятов вируса гепатита В в африканских регионах показал наличие трех субгенотипов-А1, А2, А3. В странах Азии распространено шесть субгенотипов В: В1-в Японии, В2-В5 и В7в Восточной Азии. Субгенотип В6 встречается среди коренных народов, проживающих в арктических регионах, в том числе на Аляс ке, севере Канады и в Гренландии. Генотип С имеет пять серотипов (С1-С5), которые распространены в Восточной и Юго-Восточной Азии. Генотип D также имеет пять субгенотипов (D1-D5), которые распространены в Африке, Индии, Средиземноморском регионе, Европе. Генотип F имеет четыре субгенотипа (F1-F4), которые циркулируют в Центральной и Южной Америке. Субгенотипы пока не выявлены среди генотипов E, G, H, I и J. На территории Российской Федерации циркулируют 3 генотипа вируса гепатита В (А, С, D). Среди них доминирует генотип D, распространенность которого составляет 88%.
