Physical activity in children with juvenile rheumatoid arthritis: Quantification and evaluation

Abstract: Objective. To measure daily physical activity in patients with juvenile rheumatoid arthritis (IRA] and in healthy controls, and to identify variables that may influence physical activity in IRA patients.Methods. Twenty-three prepubertal children, ages 5-11 years, with mild to moderate IRA and no prior exposure to systemic glucocorticosteroids, were compared to 23 healthy children of similar age. Physical activity was measured for 3 days (minimum of one weekend day] using 3 standardized methods simultaneously. … Show more

“…Some evidence reports that prepubertal children with JIA are less physically active when compared with healthy children (8). Additionally, other evidence reveals that children with JIA have moderate to severe impairment in physical fitness, as expressed by low maximal oxygen consumption (VO 2 ) (9).…”

“…These features have been shown to be the results of increased production of tumor necrosis factor ␣ and interleukin-1 (4,5). In addition, it is possible that reduced physical activity during childhood affects the anthropometric variables in children with JIA (6).…”

“…Many factors are thought to contribute to diminished bone mass in children with JRA, including the pathology of the disease itself, decreased physical activity due to pain and reduced joint motility (9), and glucocorticoid treatment for inflammation (6,10,11). Early in the course of the disease, JRA often begins as a localized inflammation, which contributes to periarticular osteopenia, a contained loss of bone mineral content (BMC) around the inflamed joint (12).…”

“…The majority of studies that have evaluated bone mass in patients with JRA have used dual x-ray absorptiometry (DXA) (5,7,8,10,11,17). Although DXA is a practical and common tool for the assessment of bone health, the application of DXA to pediatric populations has several limitations.…”
