La ecocardiografía de estrés cuenta hoy una creciente aceptación en la comunidad médica, dado su carácter no invasivo y la ausencia de irradiación, con elevado valor diagnóstico y pronóstico. Puede realizarse con diversas modalidades, esencialmente con ejercicio y fármacos. No se encuentra limitado al análisis de la motilidad parietal valorando en forma confiable la velocidad de flujo coronario, gradientes y reflujos valvulares, así como la presión en arteria pulmonar. La evaluación de la motilidad parietal puede ayudarse de la utilización con criterio del Doppler tisular y especialmente de la deformación por strain 2D. Proponemos una revisión integral de la técnica. En una primera parte se abordará la descripción general del método, los hitos históricos en su desarrollo y el estado actual del arte. A través de casos se intentará mostrar una visión integral de la respuesta cardiovascular al estrés en la enfermedad coronaria y en la evaluación de riesgo.

Background and aims: Approximately 10-30% of the patients with typical symptoms of angina pectoris have normal angiography showing normal macrovasculature. In these patients, however, the microvascular problems should be monitored. Hence, the main aim of this study is to evaluate retinal changes in normal angiographic patients.Methods: In this descriptive cross-sectional study, 60 normal angiographic patients with typical chest pain or anginal equivalents visiting Modarres Hospital Cardiology Research Center between 2018 and 2019 were enrolled and retinal changes were determined in Labbafinejad Hospital by Optical Coherence Tomography Angiography using Foveal Avascular Zone (FAZ), Superficial Vascular Density (SVD), and Deep Vascular Density (DVD).Results: The results of this study demonstrated that FAZ was normal in all subjects, but SVD and DVD were abnormal in 45% and 8.3%, respectively. Totally, 18.5% and 66.7% showed abnormal SVD among stable angina (SA) and unstable angina (UA) cases, respectively (P < 0.001). There was no statistically significant difference between abnormal DVD in SA and UA cases (P=0.058). Abnormal SVD was significantly more common among diabetic patients (P < 0.001), while DVD was not related to diabetes presence in the study population (P > 0.05). Moreover, abnormal SVD was more common among patients with chest pain (P=0.036), while there was no significant difference for DVD (P=0.371). Interestingly, abnormal ECG was associated with both abnormal DVD and SVD.Conclusions: The results of this study showed that nearly half of the patients with angina pectoris or anginal equivalents who revealed normal angiographic findings may suffer from retinal changes. Thus, retinal assessment is needed in these patients to evaluate microvascular changes.

Music with exercise could improve left ventricular performance, thereby decreasing the extent of tissue damage in the left ventricle of the heart. The study aimed to measure sports intervention with fast tempo music (Allegro) on the extent of cellular damage to the left ventricular myocardial tissue of the heart. This study used an experimental research design using a Random Controlled Group Posttest-Only Design. A control group was played Allegro music, a moderate sports group was played Allegro music, and a highintensity sports group was played Allegro music using Wistar white rats of the male sex. Sport was performed using a special treadmill for mice for ten weeks. Data analysis was performed using ANOVA test. The results of this study show that there was a difference in left ventricular wall thickness in the heart between the control group and allegro music group, moderate exercise group with allegro music, and highintensity group with allegro music (F=19,636; p=0.001). This study showed that high-intensity exercise with allegro music has the potential to improve the health of the left ventricle of the heart.
