Pituitary apoplexy following combination chemotherapy - a case report

Abstract: We report a case of apoplexy of an undiagnosed pituitary adenoma in a patient treated with intravenous combination chemotherapy for squamous cell carcinoma of the penis. Apoplexy occurred while he was receiving a second cycle of vinblastine, cisplatin and methotrexate and he presented with classic neuro-ophthalmological signs. He made a full recovery following conservative manageEndocrine-Related Cancer (1998) 5 151-153 ment with steroids alone.

“…To our knowledge, only two cases of pituitary apoplexy related to cytotoxic chemotherapy have been reported [ 4 5 ]. Pituitary apoplexy was reported in the second cycle of cisplatin, methotrexate, and vinblastine chemotherapy in a 70-year-old man with penile squamous cell carcinoma [ 5 ]. The patient had sudden, severe headache and diplopia due to left third-nerve palsy.…”

“…To our knowledge, there are only four other cases of PA associated with the administration of systemic chemotherapies [ 4 - 7 ] (Table 1 ). Consistent with the broad epidemiology of PA in general, these prior reports include two male and two female patients who ranged in age from 41 to 70 years.…”

“…To the best of our knowledge, there have been only four prior case reports of PA temporally associated with the administration of systemic chemotherapy [ 4 - 7 ]. We add to this literature by reporting the case of a patient who experienced PA one week after initiation of chemotherapy for metastatic testicular cancer.…”
