Plasma Cell Leukemia Presenting as a Chest Wall Mass: A Case Report

Abstract: Plasma cell leukemia (PCL) is an uncommon neoplasm of plasma cells, with an aggressive clinical course and poor outcome, even with current standard of care. It can occur either de novo (primary PCL) or as a progression of multiple myeloma (MM). This disease has unique diagnostic criteria but certain genetic markers and clinical features may overlap with MM. Due to the low prevalence of PCL, guidelines on its management are extrapolated from the management of MM and based on small retrospective studies and case… Show more

“…"The clinical presentation usually involves symptoms attributed to end-organ damage seen in MM (hypercalcemia, renal failure, anemia, and lytic bone lesions) or to leukemia (leukocytosis, thrombocytopenia, and organomegaly) " [2]. We report a case of PCL with an atypical presentation as a chest wall mass and discuss the diagnostic approach as well as treatment options.…”

“…MM represents 10% of hematological malignancies [ 4 ], with an incidence rate of 4.8/100,000 in the Czech Republic [ 5 ] and global age-standardized incidence rates between 0.1/100,000 and 5.3/100,000 [ 6 ]. MM is characterized by the aberrant expansion of malignant clonal plasma cells (PCs) inside the bone marrow (BM) microenvironment and the presence of abnormal monoclonal proteins in the serum and/or urine [ 7 ].…”

“…In the case of extramedullary myelomas, like PCL, epidural soft tissue masses are reported to be homogenously hypointense on T1-weighted images and hyperintense on T2-weighted images. Other reports of radiographic features of PCL include multi-detector computed tomography (CT) of the abdominal region exhibiting masses attached the chest wall12 or focal lesions of metastatic infiltration to the liver. Positron emission tomography/CT (PET/CT) imaging features have also been reported with increased glucose uptake in the vertebrae, ribs, and pelvis 10.…”
