Plasmapheresis for hypertriglyceridemia: The association between blood viscosity and triglyceride clearance rate

Abstract: Objectives Several factors in double filtration plasmapheresis (DFPP) were associated with triglyceride (TG) clearance rate. This study examines whether baseline whole blood viscosity was a predictor for efficient TG removal. Methods Adult subjects who receiving DFPP for hyperlipidemia in Taoyuan General Hospital from January 2015 to March 2018 were classified into efficient and inefficient TG removal according to TG removal rate ≥50% vs <50%. TG removal rate was defined as following formula: (pre‐apheresis … Show more

“…Although prior literature has demonstrated an increased tendency for pathologic thrombosis with increased weight, we hypothesize that our observation of increased filter failure may be related to increased total plasma volume that much pass through the filter. [6][7][8] As the plasma volume exchange prescription is based on total plasma volume, and plasma volume is in turn partially based upon a patient's weight, a larger patient would have an increased total volume of plasma needing removal across the filter membrane.…”

“…8,9 When hypertriglyceridemia is added to it, blood viscosity increases even more. 10 Total white blood cell, subtypes and their ratios (Neutrophil, Platelet, Eosinophil-Lymphocyte ratio; NLR, PLR, ELR ratio) in other blood parameters have been used as an indicator of chronic inflammation recently. [11][12][13][14][15][16] PLR is an inflammatory parameter that has been defined in recent years and can be easily calculated from a complete blood count.…”

“…In our reported case this efficacy was not achieved and several sessions had to be performed. This observation may be explained with the hyperviscous blood, which has been demonstrated to reduce the efficiency of triglyceride removal [39].…”
