Pneumonia in Childhood and Impaired Lung Function in Adults: A Longitudinal Study

Abstract: BACKGROUND: Diminished lung function and increased prevalence of asthma have been reported in children with a history of early lower respiratory illnesses (LRIs), including pneumonia. Whether these associations persist up to adulthood has not been established.

“…Low lung function in early life has been shown to track through childhood and into adulthood (3,4); furthermore, children born with low lung function have an increased risk of LRTI or wheezing illnesses in childhood (5)(6)(7)(8)(9)(10). LRTI in infancy has been associated with reduced lung function in later childhood and adulthood (11)(12)(13)(14). However, the sparse data from longitudinal studies of lung function in high-income settings suggest that the effect of respiratory tract illness on later lung function may be mediated through reduced lung function before the disease episode (9,10).…”

“…Longitudinal studies show that childhood pneumonia is associated with asthma, and with impaired lung function, which persists into adulthood [21]. The study region has one of the highest LRTI hospitalisation rates in the US [1,22], with median cost of LRTI hospitalisation for respiratory syncytial virus estimated at $22,323 [23].…”

“…Severe early-onset bronchiolitis is associated with an increased risk of asthma, especially after rhinovirus or RSV bronchiolitis, and an increased risk of asthma may persist into early adulthood [61,62]. An undefined aspect is up to which point early bronchiolitis in infancy damages normal lung development and predisposes to subsequent bronchospasm or even where certain infants have a preexisting aberration of immune response or airway function that predisposes them to severe bronchiolitis and recurrent bronchospasm [63].…”
