The patient was diagnosed with primary liver cancer featuring intrahepatic metastasis (Barcelona stage B; Stage IIB), hepatitis B infection, and post‐hepatitis B cirrhosis during the period of compensated liver function. Following a 7‐month regimen combining traditional Chinese herbal medicine (TCHM) with Renvatinib and Carrelizumab, the patient exhibited favorable tolerability, no bleeding risks, and stable tumor progression. The effectiveness of TCHM in this case was marked by significant symptom alleviation, reduced serum molecular markers, minimized adverse reactions, and obviated surgical intervention. Blood tests displayed alpha‐fetoprotein levels ranging from 8.85 to 20.65 IU/mL, with no increase in bleeding risks.

Background. Chinese herbal formulae possess promising applications in treating intrahepatic cholestasis. Objective. Our study aims to explore the protective effect of the San-Huang-Chai-Zhu formula (SHCZF) on liver injury in intrahepatic cholestasis (IC) and investigate the underlying mechanism related to mitochondrial oxidative stress. Methods. An IC rat model was established by α-naphthyl isothiocyanate induction. Hepatic histomorphology was observed through hematoxylin and eosin staining. Levels of biochemical indexes of hepatic function and oxidative stress were determined by an enzyme-linked immunosorbent assay. Cell apoptosis in liver tissues was detected by the TUNEL assay. The mRNA expression of mtDNA, SIRT1, and PGC-1α was measured by qRT-PCR, and the protein expression of Bax, Bcl-2, caspase-3, SIRT1, and PGC-1α was determined by Western blotting. Results. SHCZF treatment attenuated liver injury in IC. Levels of hepatic function parameters were decreased after SHCZF administration. In addition, the decreased level of malondialdehyde (MDA) and the increased levels of superoxide dismutase (SOD), glutathione (GSH), and adenosine triphosphate (ATP) in hepatic mitochondria confirmed that SHCZF could attenuate oxidative stress in IC. SHCZF treatment also reduced the apoptosis in the liver tissues of IC rats. Furthermore, SHCZF administration upregulated the expression of mtDNA, SIRT1, and PGC-1α in IC. Conclusions. SHCZF exerts a protective effect on liver injury in IC via alleviating SIRT1/PGC-1α-regulated mitochondrial oxidative stress.

The impact of the extraction method on the physiochemical characteristics and anti-inflammatory effect of polysaccharides from vinegar-baked Radix Bupleuri (VBCPs) was studied. Five extraction methods were employed to obtain the VBCPs: hot water extraction (HW), ultrasound-assisted extraction (UA), enzyme-assisted extraction (EA), citric acid-assisted extraction (CA), and ammonia-assisted extraction (KA). The results showed that the extraction method affects the yield, characteristics, and anti-inflammatory effect of the polysaccharides significantly. KA produced the highest yield, Ara content, and the strongest effect of enhancing IL-10 secretion. VBCP-EA exhibited the largest molecular weight (Mw), the highest Man content, and the poorest effect on inhibiting NO, VBCP-UA possessed more Gal than other VBCPs, the lowest Mw, and a comparable effect on inhibiting NO and TNF-α with VBCP-KA and VBCP-CA. All VBCP self-assembled into nanoparticles in solutions, and VBCP-KA presented the lowest particle size. The structure−activity analysis showed that Mw and Man content are negatively correlated and Ara content is positively correlated with the NO inhibition and IL-10 secretion effects; Rha and Gal A content are positively correlated and Glu is negatively correlated with the TNF-α inhibiting effect. The above results indicated that KA is an efficient method for obtaining anti-inflammatory VBCP, which provides new insight into the extraction of VBCP.
