Polytetrafluoroethylene Bicuspid Pulmonary Valve Replacement: A 5-Year Experience in 119 Patients With Congenital Heart Disease

Abstract: Midterm outcomes of hand-sewn PTFE bicuspid pulmonary valve replacement were acceptable. Long-term follow-up of this cohort is mandatory to determine the durability of this valve.

“…While sternal compression may be one of them, the experience of the surgical team and perhaps the anatomical substrate of the patients selected for PTFE bicuspid valve implantation may all play a role in the genesis of residual pulmonary regurgitation following valve implantation. However, the higher rate of pulmonary insufficiency observed in patients receiving a bicuspid PTFE pulmonary valve reported in this study 1 is in contrast to the observed rate of residual pulmonary insufficiency shown in other reports, 2 possibly indicating that there likely is a certain variability within center performance and operator experience pertaining to constructing bileaflet PTFE valves. In other words, one should not assume that the results obtained by more experienced teams in constructing PTFE valves are the same across the board, in contrast to bioprosthetic pulmonary valve implantation where outcomes are more easily reproducible.…”

“…Some proponents of a valve in the right ventricular outflow tract have had noted that the monocusp valve has not been particularly effective and started using a bicuspid valve that can be constructed of Gore‐tex or polytetrafluoroethylene membrane. Descriptive studies have demonstrated data that may indicate that such valves may outperform monocusp valves . However, no direct comparison of the two has been performed.…”

“…Descriptive studies have demonstrated data that may indicate that such valves may outperform monocusp valves. 16,17 However, no direct comparison of the two has been performed. Also, development of fibrosis of the constructed valve is thought to contribute to loss of valve function.…”

“…However, this artificial conduit was only used in a handful of heart centers outside of Japan ( 54 , 55 ). In the last decade, the application of 0.1 mm thick ePTFE for RVOT reconstruction has gradually increased in the form of bicuspid valves directly sutured into RVOT ( 56 , 57 ), or bicuspid/tricuspid valved conduits without bulging sinuses ( 32 , 58 ). Mercer et al reported a similar performance for bicuspid ePTFE valved conduits with homograft conduits in patients <2 years of age ( 59 ).…”
