Pos-587 Effect of Zinc and Vitamin C Supplementation on Oxidative Stress in Hemodialysis and Relationship With Infections: A Study of 46 Patients.

Abstract

The results show a large variation in the number of vancomycin levels taken during a treatment course with less than 50% vancomycin levels are within the target therapeutic range. This may be due to faults with the current dosing regimen, poor adherence to this regimen or incorrect use of TDM. A multidisciplinary team approach to discuss barriers was recommended contributing to current suboptimal therapy and options that may improve this; such as reviewing the current guidelines, staff education and governance, designated area to record dosing. Multidisciplinary meetings between Renal medicine, Infectious diseases and Pharmacy as result have developed our local line sepsis guidelines as a result of the findings and implemented changes. The first 10 patients will be included in our re-audit to close the loop and to inform any required adjustments to the new Guidelines.
