Position Paper on Post-Traumatic Headache: The Relationship Between Head Trauma, Stress Disorder, and Migraine

Abstract: Traumatic brain injury (mTBI) is a major public health concern, with mild TBI (mTBI) constituting the vast majority of the injuries. Posttraumatic headache (PTH) is one of the most frequent symptoms that follow a mTBI, occurring in isolation with a tension-type or migraine phenotype, or more often as part of a complex neurobehavioural array of symptoms. The existence of PTH as a separate entity from the primary headaches is still a matter of debate. Classification issues and a lack of methodologically robust e… Show more

“…Thus, pain is difficult to evaluate objectively because of its multifactorial nature and of the variability in the patient’s threshold. Pain threshold measurements are commonly performed by pressure, electrical or thermal responsiveness or by delivering self-reported questionnaires [ 80 , 81 ] of which the most frequently used are the WOMAC, VAS or Likert scales. Diagnostic imaging is recommended to confirm the diagnosis of OA, even though it has poor correlation with clinical pain experienced by the patient [ 82 ].…”

“…Recent structural and functional neuroimaging data have revealed that different networks other than migraine exist in persistent PTH [183]. Furthermore, calcitonin generelated peptide gene expression level is not elevated in persistent PTH, but monoclonal antibodies against calcitonin generelated peptide gene can be effective [183]. Future results from trials assessing the efficacy of such medications can provide novel treatments for PTH.…”

“…The association between migraine and psychiatric comorbidities are complex, and influence the clinical course, treatment response and clinical outcomes [15,16]. The complexity is highlighted in a recent review regrding the impact of traumatic life events and posttraumatic stress disorder on migraine patients [16].…”
