OBJECTIVE To assess the accuracy of the cervical phosphorylated insulin-like growth factor binding protein-1 (phIGFBP-1) test to predict preterm birth in women with and without symptoms of preterm labor through use of formal methods for systematic reviews and meta-analytic techniques. DATA SOURCES PubMed, Embase, Cinahl, Lilacs, and Medion (all from inception to June 20, 2015), reference lists, conference proceedings, and Google scholar. STUDY ELIGIBILITY CRITERIA Cohort or cross-sectional studies that reported on the predictive accuracy of cervical phIGFBP-1 test for preterm birth. STUDY APPRAISAL AND SYNTHESIS METHODS Two reviewers independently selected studies, assessed risk of bias, and extracted data. Summary receiver operating characteristic curves, pooled sensitivities and specificities, and summary likelihood ratios were generated. RESULTS Forty-three studies met the inclusion criteria of which 15 provided data on asymptomatic women (n=6583) and 34 on women with an episode of preterm labor (N=3620). Among asymptomatic women, the predictive accuracy of cervical phIGFBP-1 test for preterm birth at <37, <34 and <32 weeks of gestation was minimal with pooled sensitivities and specificities, and summary positive and negative likelihood ratios ranging from 14-47%, 76-93%, 1.5-4.4, and 0.6-1.0, respectively. Among women with an episode of preterm labor, the test had a low predictive performance for delivery within 7 and 14 days of testing, and preterm birth at <34 and <37 weeks of gestation with pooled sensitivities and specificities, and summary positive and negative likelihood ratios that varied between 60 and 68%, 77 and 81%, 2.7 and 3.5, and 0.4 and 0.5, respectively. A negative test result in women with an episode of preterm labor had a low to moderate accuracy to identify women who are not at risk for delivering within the next 48 hours (summary negative likelihood ratio of 0.28 in all women and 0.23 in women with singleton gestations). CONCLUSION Cervical phIGFBP-1 has potential utility to identify patients with an episode of preterm labor who will not deliver within 48 hours. However, its overall predictive ability for the identification of symptomatic and asymptomatic women at risk for preterm birth is limited.

Objectives: to determine the prevalence of the most common morbidities in extremely low birth weight (ELBW) infants hospitalized in a newborn intensive care unit (NICU) and to evaluate the influence of these morbidities through the length of in-hospital stay. Methods: observational, longitudinal, prospective and analytical study in a high risk reference maternity NICU from Sergipe, realized with 158 ELBW infants admitted between March 2014 and April 2015. The analysis of the hospitalization time was realized through the Kaplan-Meier method. Results: the average weight of premature was 785,2g ± 138,2g. The gestational age vary from 22 to 35 weeks and the average was 26,8 weeks. Of those admitted at NICU, sixty three (39,9%) were discharged and 95 (60,1%) died. The time of hospitalization was influenced for morbidities as: patent ductus arteriosus (PDA), intraventricular hemorrhage and sepsis. Acute respiratory distress syndrome was the most common complication (157 - 99,4%). The incidence of persistent arterial duct, intraventricular hemorrhage, sepsis, hypothermia, hypoglycemia and retinopathy of prematurity was 39,2%, 17,1%, 32,3%,50,3%, 52,3% e 16,6% respectively. Conclusions: the morbidities from respiratory tract, cardiac, neurological and infectious were the most prevalent, whilst PDA, intraventricular hemorrhage and sepsis were the morbidities that significantly influenced the time of hospitalization.

O trabalho de parto prematuro (TPP) é um importante desafio de saúde pública em todo o mundo, com consequências significativas para a morbidade e mortalidade neonatal e infantil. Hospitais municipais desempenham um papel crucial no atendimento de gestantes de baixa renda e vulneráveis, muitas das quais estão em maior risco de partos prematuros. Diante desse cenário, esta revisão sistemática teve como objetivo investigar o panorama atual do trabalho de parto prematuro em hospitais municipais e explorar os fatores associados a esse evento. O nascimento prematuro é uma das principais causas de morbimortalidade neonatal. Mais de 50% dos nascimentos prematuros são espontâneos. Os objetivos deste estudo foram predizer fatores de risco para a ocorrência do parto prematuro, estudar inter-relações entre fatores que afetam parto prematuro. O nascimento prematuro refere-se ao nascimento de um bebê que ocorre antes das 37 semanas completas de gestação. O parto prematuro pode ainda ser categorizado como parto prematuro tardio - 34 a 36 semanas completas de gestação, moderadamente prematuro - 32 a 34 semanas completas, muito prematuro - menos de 32 semanas e extremamente prematuro - menos de 28 semanas de gestação. Foi realizada uma análise descritiva sobre o trabalho de parto prematuro em hospitais municipais: uma revisão sistemática, utilizando uma abordagem qualitativa e o método de procedimento de coleta de dados por meio da pesquisa bibliográfica. Conclui-se que o trabalho de parto prematuro continua sendo uma preocupação em hospitais municipais, afetando uma proporção significativa de gestantes atendidas nessas instituições. Diversos fatores de risco foram identificados, incluindo idade materna, baixo índice de massa corporal, infecções genitais, fatores socioeconômicos e comportamentais, entre outros. Além disso, a falta de acesso adequado a cuidados pré-natais de qualidade e intervenções adequadas foram apontados como fatores contribuintes para o aumento das taxas de TPP nesses cenários.
