Predicting, Recognizing, and Treating Right Heart Failure in Patients Undergoing Durable LVAD Therapy

Abstract: Despite advancing technology, right heart failure after left ventricular assist device implantation remains a significant source of morbidity and mortality. With the UNOS allocation policy change, a larger proportion of patients proceeding to LVAD are destination therapy and consist of an overall sicker population. Thus, a comprehensive understanding of right heart failure is critical for ensuring the ongoing success of durable LVADs. The purpose of this review is to describe the effect of LVAD implantation on… Show more

“…The risk factors associated with its early onset include clinical and echocardiographic characteristics like prior right ventricular dysfunction, as well as hemodynamic characteristics (CVP greater than 15, pulmonary artery pulsatility index less than 1.85), among others 41 . There are also factors that favor the onset of late RHF, like arrhythmias, valve disease, the progression of right ventricular dysfunction and pulmonary hypertension 40 .…”

“…Los factores de riesgo asociados con su aparición temprana incluyen características clínicas y ecocardiográficas, como disfunción previa del ventrículo derecho, así como características hemodinámicas (PVC mayor a 15, índice de pulsatilidad de la arteria pulmonar menor a 1.85), entre otros 41 . Además, existen factores que favorecen la aparición de RHF tardía, como arritmias, valvulopatía, progresión de la disfunción del ventrículo derecho e hipertensión pulmonar 40 .…”

“…Right ventricular failure remains a common early complication following LVAD implantation, and is associated with prolonged intensive care stays and increased mortality ( 89 ). Right heart failure following LVAD arises from a multitude of factors.…”
