Prediction Models for Insulin Resistance in Girls with Premature Adrenarche

Abstract: Aim: The purpose of this study was to develop an accurate regression model to predict insulin resistance in girls with premature adrenarche. Methods: The insulin sensitivity index was calculated from the frequently sampled intravenous glucose tolerance test with tolbutamide. Thirty-five prepubertal girls (23 Caribbean-Hispanic and 12 African-American; mean age 6.8 years) were studied. The insulin sensitivity index was compared to birth weight, body mass index (BMI), the presence of acanthosis nigricans (AN), i… Show more

“…Similar findings have been reported in a population of Caribbean-Hispanic and African-American girls in the United States with LBW in girls with premature adrenarche 23. In this group, the presence of acanthosis nigricans and higher serum concentrations of 17-OHP had the strongest negative relationship with insulin sensitivity and predicted increased cardiometabolic risk 23.…”

“…In this group, the presence of acanthosis nigricans and higher serum concentrations of 17-OHP had the strongest negative relationship with insulin sensitivity and predicted increased cardiometabolic risk 23. However, in other populations, the association with LBW, insulin resistance and premature adrenarche is less clear 16.…”

“…We searched the literature to determine how the fit of this model compared to other predictive models relying on continuous outcomes such as MD and identified a range of R 2 ranging from 0.45 to 0.76. 22,23 Other examples we found had a fit considerably lower than these. While we do not claim that this is an exhaustive review of the predictive literature in medicine, we believe that this indicates that our model well meets most accepted standards of prediction.…”

“…Indeed, polycystic-appearing ovaries have been found in girls as young as 6 years [42]. Furthermore, an altered intrauterine milieu has been implicated, particularly its metabolic component [20]. However, diagnosing the disorder before puberty is difficult because patients with PCOS are generally diagnosed only after seeking help for irregular menses or skin changes that do not take place until puberty [43].…”
