Prediction of Incontinence after Robot-Assisted Radical Prostatectomy: Development and Validation of a 24-Month Incontinence Nomogram

Abstract: Incontinence after robot-assisted radical prostatectomy (RARP) is feared by most patients with prostate cancer. Many risk factors for incontinence after RARP are known, but a paucity of data integrates them. Prospectively acquired data from 680 men who underwent RARP January 2008–December 2015 and met inclusion/exclusion criteria were queried retrospectively and then divided into model development (80%) and validation (20%) cohorts. The UCLA-PCI-Short Form-v2 Urinary Function questionnaire was used to categori… Show more

“…Many papers have predictive factors for recovery of urinary continence post prostatectomy, with old age, less nerve-sparing and high BMI resulting in worse outcomes [ 2 – 4 , 9 , 14 ]. Nomograms predicting recovery of urinary continence post-RALP have been designed at 4- and 12-weeks [ 31 ] and 6-, 12- and 24-month [ 32 ] follow-up points. Pre-operative predictive measures of continence such as mean urethral length (MUL) have been described since 2002 when Coakley et al .…”

“…Their dataset includes clinical and demographic data of 680 men and their LR model achieved AUCs between 0.64 and 0.80 across 24 months. No overlap in predictors was found and the study lacked external validation ( 24 ).…”

“…The identification of the key factors (age, race, height and weight, lifestyle) that predict presentation of UI and recovery of urinary control after RP could be a very useful tool in clinical practice. 47 , 116 In this sense the development of easy-to-use nomograms allows the urologists to create a better therapeutic pathway for the continence recovery after RP. 47 , 116 …”
