Prediction of Postoperative Ileus in Patients With Colorectal Cancer by Preoperative Gut Microbiota

Abstract: BackgroundIleus and postoperative ileus (POI) are common complications of colorectal cancer (CRC). However, little is known about the gut microbiota associated with ileus.MethodDifferences in gut microbiota were evaluated by 16S rRNA gene sequencing. We characterized the gut microbiota in 85 CRC patients (cohort 1) and detected differences, and an independent cohort composed of 38 CRC patients (cohort 2) was used to evaluate the results.ResultsThe gut microbiota of CRC patients with and without ileus exhibited… Show more

“…Currently, very few clinical studies have linked gut microbiota with SSI or POI-related bacteria in CRC patients after surgery. Only one recent clinical study reported Faecalibacterium in mucosal samples from CRC patients as a possible predictive biomarker of POI [ 98 ]. Ohigashi et al is the only clinical report to identify causative fecal bacteria enriched in SSI after CRC surgery: S. aureus , P. aeruginosa , and Enterococcus spp.…”

“…Despite standard administration of perioperative antibiotics, significant changes of gut microbiota can be observed in surgical patients: bowel obstruction, for example due to colorectal carcinoma, is associated with a change of gut microbiota composition [43,44]. The aforementioned study by Hegde et al observed that the diversity measured by Shannon Index was higher in rats with bowel obstruction than in rats without.…”

“…Another trial by Jin et al reported the results of a prospective cohort study evaluating the gut microbiota of colorectal cancer patients with pre-or post-operative ileus compared to patients with no ileus [44]. They found significant differences of diversity measured by Shannon Index and UniFrac in patients with and without ileus.…”

“…Similar to existing studies [24][25][26][27][28] , 16S rRNA gene (V3-V4 region) sequencing was used to investigate the microbiota content, although it may not be possible to achieve specieslevel differentiation. We formalized the task of CRC or adenoma prediction as a classification problem and focused on operational classification units (OTUs).…”
