Predictors of Gastrointestinal Bleeding in Patients Following Left Ventricular Assist Device Implantation: A Systematic Review and meta-analysis

Abstract: Aim: Our study aims to provide a more holistic understanding of the available data and predictive risk factors for gastrointestinal bleed (GIB). Materials & methods: We searched MEDLINE, Embase, Cochrane Central Register of Controlled Trials and Web of Science Core Collection and calculated relative risk and meta-regression was utilized to evaluate for risk factors in order to assess the effect of covariates. Results: Our meta-analysis reported a pooled prevalence rate of GIB of 24.4%. Meta-regression anal… Show more

“…Common predictors in current risk models include higher age, hypertension, (chronic) kidney disease, history of stroke, prior bleeding, and anemia. Interestingly, a recent systematic review and meta-analysis found no significant associations between GIB and common predictors (eg, age, sex, hypertension, chronic kidney disease, and diabetes) in LVAD recipients [ 33 ]. This observation, together with the remarkable differences in predictor distribution within the derivation cohorts as compared with the present LVAD cohort, lead us to question which factors genuinely predict MB in patients with an LVAD.…”

“…In previous studies, the incidence of bleeding complications occurred in approximately one-third of patients discharged on dual antiplatelet therapy, and the incidence of GI bleeding was estimated to be 0.25% to 7.63% [ 29 ]. GI bleeding plays an important role in the prognosis of patients with ACS, and DM, CKD, and advanced age are important factors of bleeding complications and GI bleeding [ 30 , 31 ]. Bleeding events can impact duration and potency of antiplatelet therapy use.…”
