Aims/hypothesis Adherence to glucose-lowering drug (GLD) treatment regimens is crucial for metabolic control and improving prognosis. Because a diagnosis of cancer might have an impact on adherence to medication, this study explored changes in adherence to GLDs following a cancer diagnosis.Methods All new users of GLDs between 1998 and 2011 who lived in the Eindhoven Cancer Registry-PHARMO Database Network catchment area were selected. Those with a primary cancer diagnosis during follow-up were considered cases and matched with eligible controls without cancer during followup. Medication possession ratio (MPR) was used as indicator for medication adherence. Segmented linear auto-regression analysis with interrupted time series was used to assess changes in MPR for cases compared with controls (i.e. overall trend) due to (any) cancer diagnosis and specific cancer types. Results From the 52,228 GLD users selected, 3,281 cases with cancer and 12,891 controls without cancer during follow-up were included in the study. In our analyses, before cancer diagnosis the MPR increased by 0.10% per month (95% CI 0.10, 0.10). Besides a significant drop in MPR at the time of cancer diagnosis of −6.3% (95% CI −6.5, −6.0), there was an ongoing, yet lower, monthly decline in MPR (−0.20%; 95% CI −0.21, −0.20) after cancer diagnosis. The largest drops in MPR at the time of cancer diagnosis, in the range of 11-15%, were seen among patients with stage IV disease and gastrointestinal or pulmonary cancers. Conclusions/interpretation Our findings indicate a clear decline in adherence to GLD treatment regimens following a cancer diagnosis. The reason for the decline in MPR needs to be further elucidated.

Objective: To evaluate adherence to therapy and study factors associated with non-adherence in patients of type 2 diabetes mellitus (DM). Methods: A prospective, cross-sectional, questionnaire based study was conducted in a tertiary care teaching hospital. 150 patients suffering from type 2 DM since six months or more were interviewed using self-designed, semistructured questionnaire to get information about adherence to medication, diet/exercise schedule and self-monitoring of blood glucose. Morisky medication adherence questionnaire was used to calculate overall adherence. Results: Average number of medicines taken by each patient daily was 4.75 ± 1.73 (mean + SD). 70% patients reported non-adherence to medication schedule. Not buying all medicines (58.66%), not taking prescribed dose of medicines (34%), taking additional non-prescribed medicines (30%) and not taking medicines for required duration (25.33%) were common types of nonadherence. Unawareness about need of each medicine (55.66%), forgetfulness (50.66%) and high cost (43.33%) were the common causes of non-adherence. In Morisky medication adherence questionnaire high, medium and low adherence was reported by 0%, 26% and 74% of patients, respectively. Moderately positive correlation was observed between poor adherence and number of concurrent illnesses, number of medicines taken and female gender. Regular monitoring of blood glucose and diet schedule was followed by 46% and 68% patients respectively. Conclusion: Since, not buying all medicines, which was mostly due to high cost was the commonest type of non-adherence, measures to provide free medicines to non-affording patients need to be implemented. Majority of patients were not aware about need of taking each medicine, hence, creating awareness about the same is essential for improving adherence to therapy.

Objective: To report medication adherence among ambulatory patients with diabetes mellitus (DM) using Morisky 8-item medication adherence MMAS-8 scale and assess current standard of knowledge regarding their disease using a especially developed Patient 10-item Knowledge Assessment PKA-X scale. Methods: A quantitative cross sectional study was conducted for 3 months in Karachi, Pakistan using Morisky 8-item medication adherence scale ® documenting the medication adherence of ambulatory patients with DM and to find out their knowledge regarding the disease using a newly developed Patient 10-item knowledge assessment PKA-X scale. Results: The mean MMAS-8 score of the total sample was 4.69 (1.9 SD) which was interpreted as 'Low medication adherence' (P value<0.01). Majority of patients (N=204, 79.4%) had low adherence (P value<0.01). The mean score reported by PKA-X scale was 9.0 (SD 1.4) which was interpreted as 'Excellent knowledge'. Bulk of patients (N=202, 78.6%) had excellent knowledge (P value<0.01). No significant association existed between patient knowledge and their medication adherence (P value>0.05). Conclusion: The medication adherence of the patients is very low and adequate measures are the need of the hour to address this issue though the standard of knowledge has greatly improved. However, having good knowledge about the disease does not guarantee adherence to medication regimen.
