Predictors of Nursing Home Admission for Persons with Dementia

Abstract: Several results seemed to challenge conventional assumptions of what precipitates NHA among persons with dementia. Caregiver stressors in conjunction with care recipient characteristics are important to consider when assessing NHA risk. The findings emphasize the need to construct more complex models of institutionalization when designing risk measures to target interventions.

“…Whereas many previous studies show caregiver burden to be a strong predictor for institutionalization, 13,14,29,30 only 15% of caregivers actually declare their burden as a reason for institutionalization. This is an interesting result because it may imply that caregivers do not (always) label what is considered 'burden' by formal caregivers as such.…”

“…Current knowledge regarding reasons for institutionalization is mainly derived from studies on predictors of institutionalization, based on the results of standardized instruments. [13][14][15][16] More insight is needed regarding the perspective of the informal caregiver in order to understand their experiences and anticipate institutionalization to complement our current knowledge.…”

“…Most research in the past decade has shown that informal caregivers have more health problems, visit healthcare professionals more frequently, suffer from isolation, and have an increased risk of depression, distress, and other illness [1,2,6,[11][12][13][14][15]. Because caregiver burden can reduce quality of caregiving and increases the likelihood of care-recipient placement into institutional care, there is also a substantial risk for the care-recipient [6,16,17].…”

“…1 This trend is alarming because cognition is the most important determinant of health status, quality of life, and functional ability in older age. 2 Diminished cognitive capacity causes significant psychological, social, and economic hardship and adversely impacts a person's ability to benefit from treatment for other medical problems. 3 Due to the current paucity of treatments for dementia, management of treatable factors that place the brain at risk for subsequent cognitive decline is the primary line of defense, especially in cases in which vascular factors play a key role.…”
