Predictors of spontaneous bleeding in Dengue

Abstract: Prothombin time was abnormal only in cases with spontancous bleed. A combination of (a) biphasic pattern of fever, (b) hemoconcentration, (c) platelet count less than 50,000/mm3 and (d) elevated ALT had a sensitivity of 79.2%, specificity of 64.7% with a positive predictive value of 70% and a negative predictive value of 75% in predicting spontaneous bleeding in dengue.

“…Particularly, adult patients may present with bleeding than children and bleeding manifestation may occur in the absence of plasma leakage. (4) This observation raises 2 questions that bleeding and plasma leakage may be 2 independent complications. Thrombocytopenia is strongly associated with bleeding.…”

“…Bleeding manifestations are highly variable and do not always correlate with the laboratory abnormalities in the coagulation profile. Factors like mild degree of disseminated intravascular coagulation (DIC), hepatic derangement and thrombocytopenia act synergistically to cause bleeding in dengue patient 13 . Severe bleeding is related to severe thrombocytopenia 14 .…”

“…Bleeding in dengue can be caused due to Disseminated Intravascular Coagulation (DIC), hepatic derangement and also due to severe thrombocytopenia. 2 In the present study, the patients have been classified into dengue without warning signs, dengue with warning signs and severe dengue according to the WHO bulletin, 2009. 3 Prophylactic platelet transfusion is recommended in patients with platelet count less than 10000/cumm and platelet transfusion is advised in patients with associated bleeding or hemorrhagic manifestation along with thrombocytopenia (count more than 10000/cumm).…”
