Predictors of Survival 1 Hour After Implantation of an Intra-aortic Balloon Pump in Cardiac Surgery

Abstract: From July 1996 to March 2000, 391 patients with intraoperative cardiac low-output syndrome who underwent surgery with heart-lung bypass and had an intra-aortic balloon pump (IABP) implanted were analyzed in a prospective study. Of these 391 patients, 153 (39%) were operated on in an emergency situation, and 238 (61%) patients had elective surgeries. The perioperative mortality was 34% (133 patients). Clinical parameters were analyzed 1 hour after IABP support began. Statistical multivariate analysis showed tha… Show more

“…3,9,18,22 Cardiac failure appears to be relevant only in the first post-operative days, when it is described as postcardiotomy cardiogenic shock. 19 However, a protracted inflammatory response with high catecholamine dependency after cardiac surgery could also lead to other organ failures, such as ARF-D and respiratory failure. 5 -7 Measures to reduce the risk of cardiac and non-cardiac organ failures should therefore be taken throughout the entire hospital stay, ideally early on or as a pre-operative preventive measure.…”

“…In patients whose ICU stay was > 14 days, the stay was shorter in the patients with a VAD (median [25% quartile, 75% quartile], 23 [19,37]) days) than in those without (28 [21,40] …”

“…63,64 This state may be attributable to low cardiac output (a result in part of myocardial hibernation, stunning, or inadequate cardioprotection), systemic vasodilation, or both. [63][64][65] If these common causes of CS are not consistent with the presentation, then less common causes listed in Supplemental Table 1 should be considered. In acute e236 myocarditis, paradoxically, the sickest patients on presentation have the best odds of recovery, particularly in younger age groups.…”

“…IAB has been recommended for patients with obstruction of left coronary branch 165 in mitral insufficiency in a condition of postoperative low output 166 . In patients whose signs of left ventricular dysfunction and tissue hypoperfusion are intense since the beginning, or in where the IAB failed to restore tissue perfusion, the implant of artificial ventricular support must be considered 167,168 .…”
