Pregnancy-associated breast cancer: maternal breast cancer survival over 10 years and obstetrical outcome at a university centre of women’s health

Abstract: The present findings on survival rates suggest that maternal medical assessment at the beginning of and during further course of pregnancy should include a scrutinized thorough breast examination. Conveying/delivering special competences to monitor these high-risk pregnancies at the interface of oncological care should be considered an obligatory part of academic medical education, obstetrical training and interprofessional midwifery education.

“…The definition of PABC varies, but it is generally defined as during pregnancy and within one year of delivery. A high frequency of higher grade, triple-negative breast cancers, and HER2+breast cancer was observed in PABC, and 5-year OS was worse (42)(43)(44). In a meta-analysis, PABC had a higher risk of death (OS) than non-PABC (HR 1.44, 95% CI 1.27-1.63) and postpartum breast cancer had poorer outcomes than breast cancer during pregnancy (45).…”

“…It states, for example, that graduates should have “evidence-based knowledge and skills to promote physiological pregnancy, birth and puerperium”, but at the same time they should “recognise signs of irregularities that require medical treatment and cooperate with doctors and other professional groups in the planning, organisation, implementation, control and evaluation of care processes for women and their families with a pathological course during pregnancy, birth and the puerperium” 11 . In order to meet these requirements of interdisciplinary teamwork, students need to be sensitized to the necessity of interdisciplinary cooperation during their course of study 12 . This requires the simultaneous development of future-oriented teaching formats which also convey scientific and medical basics and are as interprofessional as possible, including students of other subjects of a medical faculty 3 , 4 , 5 , 7 , 9 .…”

“…According to the German Qualifications Framework, college degrees are classified as level 4 whereas Bachelor degrees are classified as level 6 5 . The rationale behind the proposed academization of the midwifery profession and the switch to university-based training is that this takes account of midwivesʼ increasingly complex and continually expanding scope of duties 6 and provides them with additional skills 7 . Just one year before the Directive must be completely transposed into national law, it is still not clear when complete academization can be achieved in Germany.…”
