Preliminary evaluation of intratumoral injection of a streptococcus pyrogenes preparation in patients with malignant brain tumors

Abstract: Thirteen patients with malignant brain tumors, 12 anaplastic gliomas and one metastatic tumor, received repeated intratumoral injections of an immunopotentiator, Picibanil, prepared from streptococcus pyogenes. All patients tolerated this therapy; morbidity rates were acceptable. Significant tumor regression was noted on computerized tomography scanning for 6 of 12 patients for whom scanning was performed. Histologic examination of the post-therapy specimens obtained from 8 patients revealed that inflammatory … Show more

“…Antitumor effects of intratumoral administration of OK-432 have been significantly shown in patients with breast and maxillar cancer. When administered intratumorally, OK-432 can induce local inflammatory granulomatous reactions, as has been shown in Calmette-Guerin (BCG) bacillus-injected tumors [7]. Furthermore, intracerebral injection of OK-432 in patients with malignant brain tumors induced inflammatory reactions without causing neurotoxic effects on the central nervous system.…”

“…OK-432, which was used in case 3, has been shown to have antitumor activity in cancer patients [7,8]. Systemic administration of OK-432 increases the activity of blood natural killer (NK) cells, which play an important role in host resistance against tumors.…”

“…The divergent results in the statistical tests (Log-rank vs. Gehan-Breslow-Wilcoxon) suggest that the potential effect of SSIs applies more to early recurrences and is limited in time. The hypothesized mechanisms of a potential survival benefit of SSI patients relate to local processes including a local immune reaction (competition between recruited immune cells and tumor cells for proteins and nutrients), activation of anti-tumor immune cells and release of specific cytokines or potential bacterial anti-cancer toxins (De Bonis et al 2011 ; Hayes et al 1995 ; Kazim et al 2021 ; Löhr et al 2013 ; Tanaka et al 1980 ). Furthermore, it has recently been demonstrated, that bacterial peptides might activate tumor-infiltrating T-cells in glioblastoma and lead to an anti-tumor effect (Naghavian et al 2023 ).…”

“…For instance, a study reported that intracerebrally implanted heat-inactivated staphylococcal epitopes mixed with 9L gliosarcoma cells in Wistar rats, led to significantly prolonged survival than experimental controls (46). In a study by Tanaka and colleagues, Picibanil, also called OK-432 (an immunopotentiator and a low virulent mutant strain of Lancefield's Type III, Group A Streptococcus pyogenes) was injected intratumorally in 13 patients with malignant brain tumors, resulting in significant tumor regression in 50% of patients (47). Further indirect evidence comes from basic science studies of antitumor effects of bacterial toxins.…”
