Prenatal pelvic MRI: Additional clues for assessment of urogenital obstructive anomalies

Abstract: Third trimester fetal MRI is an essential exam for characterization of pelvic cystic mass diagnosed by ultrasound. This exam appears valuable for invalidating the diagnosis of cloacal malformation and for differentiating between isolated genital obstruction and urogenital sinus.

“…[ 13 ] CM is commonly found in female newborns. [ 2 , 3 , 5 , 6 , 11 , 13 , 14 , 17 ] As far as we know, only 2 male neonates have been reported in the literature. “…This may be because the genital tract and urinary tract are interconnected due to the developmental malformation, ultimately resulting in communication signals. [ 14 ] In this study, we can clearly observe the positional relationship among intestine, vagina, and bladder with the aid of MRI. Therefore, MRI is beneficial to evaluating pelvic anatomy, rectal contents, and abnormalities when the prenatal US cannot make a definitive diagnosis.…”

“…Next to ruling out associated anomalies or absence of the rectum, estimating of the length of the common channel may be helpful for the pediatric surgeon in parental counseling, given it affects urinary prognosis . The use of fetal MRI in simple conditions, such as isolated hypospadias is questionable, since it does not impact management nor counseling …”

“…Although rare, pelvic cystic masses can lead to a suspicion of an obstructive urogenital malformation . These malformations include obstructive uropathy, obstructive genital anomaly, combined obstructive urogenital anomaly, or cloacal anomalies . These anomalies represent a range of different pathologies, each carrying very different prognosis.…”

“…Ultrasound (US) examination has proven insufficient to distinguish isolated hydrocolpos from cloacal dysgenesis, as these malformations display similar prenatal patterns . Recent literature has emphasized the potential role of fetal magnetic resonance imaging (MRI) in urogenital malformations . Indeed, it is highly important to identify cloacal malformations, as they carry a much poorer prognosis.…”
