“…The purpose of all the minimal modifications was to provide a tension-free, well-vascularized tubularized neourethra and improve postoperative wound healing (2). The existing modifications were only designed to improve external condition, such as soft tissue interposition, removal, increase length and width of rectangular flap and in situ tubularization of CES = continuous eversion suture; CIS = continuous inversion subepithelial; UF = cutaneous fistula; UD = urethral diverticulum; US = urethral stricture and BU = breakdown of urethral repair the transverse preputial island flap (3,9). Previous studies only mentioned that a well-vascularized neourethra and preputial flaps used for repair were exceptionally important for a successful outcome (10), but neglected how to improve intrinsic element of neourethra.…”

“…Previous literature has identified that proximal hypospadias, a more severe phenotype of the condition, is linked to higher rates of complications post-operatively (19,20). Likewise, the hypospadias severity is an important confounding variable to consider a subgroup analysis to actually identify the causality or correlation of an intervention (21). In the choice of anesthesia, the subpopulations of proximal hypospadias are more likely to be given with caudal anesthesia over other anesthesia approach due to its more complex repair and long duration procedure.…”

“…The current clinical practice guidelines published by the European Association of Urology and the European Association of Pediatric Urology provide no strong statement of recommendation on PHS with androgens as data are scarce on this specific issue. 1,2 To date, three systematic reviews have been published on the effects of using PHS for hypospadias repair on post-operative complications. In the first two systematic reviews, the odds ratios for a complication occurring with pre-operative hormonal stimulation use were reported as 1.67 (CI: 0.96-2.91, p = 0.07, I(2) = 0%) and 1.18 (95% CI: 0.70-2.00, Z = 0.91, p = 0.539) respectively.…”

“…A pooled effect estimate showed no significant difference between the treatment groups (RR 0.99, 95% CI: 0.63-1.55, p = 0.96). 1 As a result of these evaluations, it has been reported that there is a need for future studies on the molecular effects of testosterone at tissue level wound healing.…”
