Preoperative pregabalin administration significantly reduces postoperative opioid consumption and mechanical hyperalgesia after transperitoneal nephrectomy

Abstract: Our study has shown that preoperative administration of 300 mg pregabalin in patients undergoing transperitoneal nephrectomy reduces postoperative opioid consumption and decreases the area of mechanical hyperalgesia.

“…6,39,60,68,73,80,112,133,134 Along with opioid, neurokinin 1, and toll-like receptor 4 antagonists, NSAIDS and gabapentinoids have been found to reduce neuroinflammation and mitigate OIH and/or WIH in some 1,5,14,18,34,45,46,61,70,83,85,94,100,121,127,135,137 but not all 50 circumstances. In our study, a subset of participants named NSAIDs most frequently and gabapentinoids third for relieving WISP, supporting a possible neuroinflammatory component.…”

“…Remifentanil at rate of 1 g kg −1 min −1 for 60 min used in this study depended predominantly on clinical practice and has been shown to induce hyperalgesia by our previous studies (Bornemann-Cimenti et al, 2012;Yuan et al, 2013). Sevoflurane was selected for anesthesia in that it was previously revealed without influence on pronociceptive thresholds (Celerier et al, 2006).…”

“…Numerous articles have manifested that remifentanil, a short-acting selective MOR agonist, at clinically relevant dose as an intraoperative analgesic could cause pain hypersensitivity and enhance postoperative analgesics consumption (Bornemann-Cimenti et al, 2012;Ishida et al, 2012).…”

“…Strategies for preventing, reversing, or managing OIH have been attempted [1,2,9,11,12], and pregabalin has recently emerged as an effective treatment for OIH [9,17-20]. Pregabalin binds potently to the α2δ-1 subunit of voltage-gated calcium channels, which may impair channel trafficking and reduce the release of several neurotransmitters, including glutamate, noradrenaline, serotonin, dopamine, and substance P. This also results in interactions with spino-bulbo-spinal loop-comprising projection neurons in the superficial dorsal horn and brainstem, causing 5-hydroxytryptamine3 receptor-mediated facilitation [17,18].…”
