Presence of antimitochondrial autoantibodies in patients with autoimmune hepatitis

Abstract: Antimitochondrial autoantibodies were found to be present in sera of patients with AIH more frequently than expected, even at very low titer. However, clinical features and histological findings of AIH were not influenced by the AMA status.

“…These results suggested that PBC patients representing with negative AMA is also a subpopulation of AMA-positive PBC patients in China. Surprisingly, 15.9% of the AIH patients we studied were AMA-positive, and that was lower than the 34% positive value described in a Japanese population [20]. Based on a previous study that described fluctuating AMA levels in AIH patients over time [21], we considered this possibility in the Chinese patients described here as an explanation for the reduced levels observed.…”

“…Disease controls consisted of 5 groups: autoimmune hepatitis (AIH) (281), primary sclerosing cholangitis (PSC) (77), viral hepatitis (189), other liver diseases (31), and other infectious or autoimmune diseases (AID) (30). The diagnosis of AIH was made according to internationally recognized criteria [19]; the diagnosis of PSC was established based on diagnostic features shown by endoscopic retrograde cholangiography or magnetic resonance cholangiopancreatograpgy [5,20].…”

“…The universal availability and superior sensitivity of assays for ANA, SMA, and anti-LKM1 for autoimmune hepatitis justify their use as the first-line testing battery [1,10]. Antimitochondrial antibodies (AMA) are highly specific serological markers for primary biliary cirrhosis (PBC) [10], but they can occur in otherwise classical autoimmune hepatitis [34][35][36][37][38][39]. To avoid diagnostic confusion, the absence of AMA is an appropriate diagnostic requisite for autoantibody-negative autoimmune hepatitis in addition to the absence of ANA, SMA, and anti-LKM1.…”
