“…Unmeasured reasons that physicians may choose clindamycin include (1) a mistaken belief that it is adequate coverage for MRSA, 15 (2) its simple dosing compared to antibiotics that must be adjusted in renal and hepatic impairment, (3) its ease of transition to oral regimens, and (4) its lack of monitoring requirements, as is needed for vancomycin. If SCIP or other quality guidelines are to be adopted for head and neck surgery, these data and data from previous trials suggest that the ASHP-recommended ampicillin/sulbactam and cefazolin 1 metronidazole are good candidates.…”

“…Reports of DAP-nonsusceptible enterococcal strains, defined as an MIC of Ͼ4 g/ml, are becoming more prevalent, however (16). Recent data from a single center demonstrate that up to 0.5% of E. faecalis and 4.7% of E. faecium strains possess DAP MICs of 4 g/ml (17). At these MICs, mutations that confer DAP inactivity are more prevalent than at lower MIC values, necessitating caution when treating these isolates (18).…”
