Diabetic nephropathy (DN) is the most common cause of end stage renal disease (ESRD) and it accounts for onethird of all patients requiring renal replacement therapy in Africa. Diabetic patients with microalbuminuria (MA) have an increased risk of progression to macroalbuminuria and later to ESRD. So detecting MA which is a marker of DN helps to alert the clinician to intervene at a time when future renal damage is still preventable. The main goal of this study was to determine the prevalence of MA in diabetic patients at the Bamenda Regional Hospital. This study was a prospective cross sectional study involving diabetic patients at the Bamenda Regional Hospital. A random (spot) or first morning urine specimen was collected and MA was measured by a semi quantitative dipstick method using the URS-14H urine test strips. The prevalence of MA in the present study was 34.6%, which is high but similar to the results of other studies done in sub-Saharan Africa. Although MA was more common (50%) in individuals with diabetes more than 16 years, a high proportion (42.85%) of patients with MA had a duration of diabetes of ≤ 5years. There was no significant difference in the occurrence of MA with respect to age, gender and duration of diabetes. This study showed a high prevalence of MA in the diabetic population presenting at the Bamenda Regional Hospital. Consequently, measures of glycemic control should be enhanced in this population to prevent the progression to macroalbuminuria and ESRD.

Microalbuminuria, for which hypertension and diabetes are well-known risk factors, has recently been used to detect individuals at risk for cardiovascular and chronic kidney diseases in the general population. We aimed to determine the thresholds of blood pressure or blood glucose concentration at which the odds ratio of having microalbuminuria begins to increase. An annual public health checkup was conducted on 3166 participants aged 29-84 years (mean, 61 ± 11 years; 40% men) living in Watari town, Japan. We studied their demographic data, medical history of hypertension, diabetes, dyslipidemia, sitting blood pressure and fasting blood samples. Urinary albumin excretion was examined in terms of the albumin/creatinine ratio in spot urine samples. Microalbuminuria was defined as 30-299 mg albumin per gram creatinine. Final analyses included 2133 participants under no anti-hypertensive or anti-diabetic medication. Microalbuminuria was detected in 118 subjects (5.5%). We used the adjusted odds ratios for microalbuminuria in different blood pressure and blood glucose groupings as the reference. The adjusted odds ratio increased linearly with increase in systolic blood pressure. A similar trend, although not as linear, was observed with diastolic blood pressure. The odds ratio increased significantly with high-normal levels of both systolic and diastolic blood pressure. For blood glucose, the adjusted odds ratio increased significantly from the impaired fasting glucose level. Therefore, the odds ratio for microalbuminuria begins to increase from high-normal blood pressure and impaired fasting glucose levels in comparison with the reference. Early intervention in such cases may be useful for reducing cardiovascular and renal risks.

ABSTRACT-Background: Microalbuminuria in hypertension has been described as an early sign of kidney damage and a predictor for end stage renal disease and cardiovascular disease. More specifically it is seen amongst patients suffering from hypertension. Methods: This study was conducted at Dr. Ram Manohar Lohia Institute of Medical Sciences, Lucknow, India in the department of emergency medicine and 84 subjects were included in the evaluation in the age of more than 30 years. All patients were diagnosed by clinical examination, anthropometric measurement, blood pressure, urinary microalbumin, and urinary creatinine. Statistical analysis was done by using SPSS, version 16.0 p-values were calculated by chi-square test, ANOVA unpaired t-test. The p <0.05 was considered statistically significant. Results: It was found that microalbuminuria among hypertensive patients increased steadily with the advancing age and the duration of hypertension. The features of high urinary microalbumin 52.09±8.62 mg/24hr and the urinary creatinine 2.37±0.86mg/dl were prevalent in hypertensive patients and it increased in both male and female patients. Conclusions: The prevalence of microalbuminuria in hypertensive individuals is high, and it revealed strong association between microalbuminuria and hypertension. Our findings suggest that microalbuminuria could be a useful marker to assess risk stratification and management of cardiovascular disease and renal disease.
