Prevalence of oral and blood oncogenic human papillomavirus biomarkers among an enriched screening population: Baseline results of the MOUTH study

Abstract: Background Human papillomavirus (HPV)‐related oropharyngeal cancer screening is being explored in research studies, but strategies to identify an appropriate population are not established. The authors evaluated whether a screening population could be enriched for participants with oncogenic HPV biomarkers using risk factors for oral HPV. Methods Participants were enrolled at Johns Hopkins Hospitals and Mount Sinai Icahn School of Medicine. Eligible participants were either men aged 30 years or older who had t… Show more

“…The specific transmission routes of HPV are also of great significance to the prevention and control of OCC. Although it can be transmitted through contact, sexual transmission, obviously including oral sex, is the main route of HPV infection ( 29 31 ). In terms of oral cavity, HPV colonizes the oral cavity in various ways.…”

“…Though there are no current recommendations for population screening for HPV-OPC, further refinement of high-risk groups ( 65 ) that may benefit from screening could potentially allow for the use of ctDNA for screening in the future. For example, a recent study applied ctDNA testing to patients with clinical findings concerning for OPC, and correctly identified HPV-OPC in a subset of patients with positive testing ( 66 ).…”

“…The pursuit of HPV testing is expressly advised in instances where histological evidence strongly indicates HPV-OED. Screening for HPV infection in isolation is discouraged due to an imperfect balance of advantages and potential drawbacks (37). This imbalance is primarily attributed to the transient nature of most oral HPV DNA, increasing the probability of false positives.…”
