Introduction: Cataract surgery is one of the most commonly performed surgical procedures worldwide. The final postoperative visual acuity is dependent on the pre-existing corneal astigmatism, accurate biometry and surgically induced astigmatism. Because of the increase in the patient’s demand for better postoperative visual acuity, it becomes important to evaluate the distribution of pre-existing corneal astigmatism in different populations. The objective of the study was to analyze the magnitude and pattern of pre-existing corneal astigmatism in cataract surgery patients in Western Nepal.Methods: A hospital-based cross-sectional descriptive study was carried out in the Department of Ophthalmology, Manipal Teaching Hospital, Pokhara. Three hundred and two patients undergoing routine cataract surgery between February 2016 and January 2018 were recruited in the study. Keratometric data from these patients were collected and were analyzed using the statistical software SPSS version 21.Results: The mean age of the subjects was 70.25 ± 10.97 years (range 35 to 94 years). There was a preponderance of females (62%) over males (38%). The mean corneal astigmatism was 0.78 ± 0.59 D (range 0.00 to 5.00 D). The pre-existing corneal cylinder ≥ 1.00 D was seen in 40.06% of patients. The corneal astigmatism was mainly against the rule (65.56%). The amount of corneal astigmatism and corneal steepness increased with age.Conclusion: Corneal astigmatism of 1.00 D or more was seen in 40.06% of the patients undergoing cataract surgery which should be corrected to meet the patient’s expectation for good, unaided visual acuity after cataract surgery.

Background: To investigate the magnitude and pattern of pre-existing corneal astigmatism in age related cataract patient. Method: This prospective observational study was carried out in a tertiary eye hospital from January 2020 to June 2020. We examined 577 eyes of 532 patients who underwent cataract surgery. The mean age at the time of surgery was 61.2 ±7.1 (40 to 100) years. Corneal astigmatism was measured with Auto Refracto Keratometer at least two times for each patient. Astigmatism was calculated from dioptric difference of vertical reading from horizontal reading. With the rule (WTR) astigmatism was considered when steep meridian at 900±200. Against the rule (ATR) astigmatism was considered when steep meridian at 1800±200. Astigmatism is in other direction is defined as oblique. Result: The pattern of astigmatism was ATR astigmatism in 237 eyes (41%), WTR astigmatism in 165 eyes (28.6%) and Oblique in 175 eyes (30.4%). Prevalence of ATR astigmatism was more than WTR astigmatism and prevalence of ATR astigmatism axis significantly increased with the increase in age. Average corneal power was 45.23D. Corneal astigmatism was 0.0D to 0.5D in 274 eyes (47.5%), 0.51D to 1.0D in 175 eyes (30.3%), 1.01D to 2.0D in 98 eyes (17.0%) and more than 2D in 30 eyes (5.2%) cases. Corneal astigmatism of most cataract surgery candidates fell between 0.50D to 1.50 D in 360 eyes (62.4%) cases. Conclusion: Thirty five percent (35%) pre-operative patient had 1D or more corneal astigmatism and sixty five percent (65%) had less than 1Dcorneal astigmatism. Purpose: The purpose of this study was to assess the prevalence of pre-operative astigmatism in patient reporting for cataract surgery using swept-source optical coherence tomography (OCT) in a tertiary care eye hospital. Methods: Prospective cross-sectional study in patients undergoing cataract surgery with having age ≥35 years. Ophthalmic biometry was done using swept-source OCT (intraocular lens, IOL Master 700). The various parameters that were studied are axial length, anterior chamber depth, lens thickness, white-to-white measurement, and keratometry (K) (K1-flat K and K2-steep K). Corneal astigmatism was determined by the difference between K2 and K1 reading. Results: A total of 310 eyes from 155 patients were evaluated. There were 70 males (45.16%) and 85 females (54.83%). The mean age of study population was 64.43 ± 9.18 years. The mean corneal astigmatism was 0.91D ± 0.67D ranging from 0 to 4.21D. No astigmatism was present in 3 eyes (0.96%), astigmatism with-the-rule was detected in 90 eyes (29.03%), against the rule astigmatism was detected in 167 eyes (53.87%), and oblique astigmatism was found in 50 eyes (16.12%). 214 eyes (69.02%) had corneal astigmatism <1.00D and 96 eyes (30.98%) had corneal astigmatism ≥1.00D. Conclusion: The study shows that 30.98% of eyes reporting for cataract surgery had pre-existing corneal astigmatism ≥1.0D and were offered the option of astigmatism correction with toric IOL implantation. The larger majority of eyes 69.02% had corneal astigmatism <1.0D and could be managed with spherical monofocal IOL.
