Preventing Pressure Ulcers in the Veterans Health Administration Using a Virtual Breakthrough Series Collaborative

Abstract: The Veterans Health Administration implemented a Virtual Breakthrough Series to prevent pressure ulcers. The pressure ulcer rate decreased from 1.2 to 0.9 per 1000 bed days of care (P = .017). The most common interventions were education (N = 26; 68%), improved documentation (N = 23; 61%), and the use of equipment and supplies (N = 21; 55%). In summary, this project helped improve pressure ulcer rates in the Veterans Health Administration and presents a promising model for implementing a virtual model for impr… Show more

“…[40][41][42][43] Virtual (internet-and/or phone-enabled) adaptations of collaboratives are increasingly used to mitigate participation barriers but often still require dedicated learning sessions and often last a year or more. [44][45][46][47][48][49][50] One large-scale quality collaborative in VHA provided virtual monthly coaching but only after teams started executing their planned PDSA cycle. 47,48 LEAP coaches, in contrast, walked teams through every step, week by week, tailoring guidance to the needs of teams and providing feedback on interim progress.…”

“…Most published studies on VLCs are descriptive and have not definitively tested its effectiveness to enhance adoption and implementation [16,[71][72][73][74][75][76][77][78][79][80][81][82]. There are also few randomized-clinical trials that evaluate traditional in-person learning collaboratives [14,15,21,22] or compare their effectiveness to other implementation approaches [16,17,71,83].…”

“…[19,26,38] A lack of recording in the results about unintended consequences, such as unexpected bene ts, problems, failures, or costs associated with the intervention or details about lost data, was observed in 38% of the studies. Thus, 44.1% of the articles were classi ed as high quality with 5 to 6 Squire items; [15,[21][22][23]25,26,28,29,34,38,39,41,42,45,48] 44.1% were classi ed as average quality with 3 to 4 Squire items [16][17][18][19][20]24,27,[30][31][32][33]37,40,43,44] and 11.8% were classi ed as low quality with 1 to 2 Squire items. “…International patient safety goals were addressed in 10 studies, ve focused on improved communication between teams and/or with the patient/family, [19,20,26,37,39] three addressed falls reduction, [35,36,47] and two considered health care-related infection prevention. [30,38] Improvement of patient safety culture was the subject of three studies.…”
