Prevention of Venous Thromboembolism in Microvascular Surgery Patients Using Weight-Based Unfractionated Heparin Infusions

Abstract: Background Unfractionated heparin infusions are commonly used in microvascular surgery to prevent microvascular thrombosis. Previously, fixed-dose heparin infusions were believed to provide sufficient venous thromboembolism (VTE) prophylaxis; however, we now know that this practice is inadequate for the majority of patients. Anti-factor Xa (aFXa) level is a measure of unfractionated heparin efficacy and safety. This study evaluated the pharmacodynamics of weight-based dose heparin infusions and the impacts of … Show more

“…Bertolaccini et al used a combined approach with weight-based dosing and dose adjustments for the patients that did not reach in-range afXa levels in the microsurgical patient population. 39 While some patients reached target levels, most required dose adjustments. Based on the previous studies, modifications may be extrapolated for the microsurgical free flap population.…”

“…With the majority of patients still requiring dose adjustments after weight-based infusions, this study highlighted variations in individual pharmacodynamics and therefore the need for individualized chemoprophylaxis regimens. 34 Clinically, VTE prevention has also been trending toward individualization in plastic surgery. In 2016, the consensus conference hosted by the American Association of Plastic Surgeons recommended consideration of chemoprophylaxis on a case-by-case basis in patients with a Caprini score greater than 8.…”
