Preventive effect of lacrimal occlusion on topical timolol‐induced bronchoconstriction in asthmatics

Abstract: Lacrimal occlusion with intracanalicular collagen plugs may almost completely prevent the bronchoconstriction caused by topical timolol in asthmatics by inhibiting or decreasing systemic absorption of the medication.

“…This route of systemic absorption of biologically active compound avoids the first‐pass effect normally observed after oral presentation of a compound, and the pharmacological sequelae resemble those seen after an intravenous administration. This pharmacokinetic phenomenon has been given in clinical situations with topical administration of antiglaucoma medicines to the eye, because the systemic effects of drugs such as timolol can be quite pronounced [56].…”

“…Plasma levels of ocular drugs are lower when lacrimal occlusion is applied with punctal occlusion or with fi nger pressure immediately after installation of any eyedrop. 21,22 In conclusion, glaucoma treatment with latanoprost seems to be safer in elderly people in comparison with the timolol treatment. Even if they have no history of airways disease, some deterioration can occur in the respiratory and cardiovascular systems due to beta-blocker therapy.…”

“…Nasolacrimal duct occlusion by hand or by silicone plugs has been shown to increase the therapeutic index of several glaucoma drugs 19 and decrease their systemic effects. 20,21 Using timolol in a gel formulation allowed once daily dosing 22 with decreased systemic absorption. 23 The Corneal Barrier…”
