Effects of Timolol and Latanoprost on Respiratory and Cardiovascular Status in Elderly Patients With Glaucoma

Abstract: Although all the results presented were statistically insignificant, switching to latanoprost treatment may offer some advantages in respiratory and cardiovascular function for elderly people with glaucoma over beta-blocker drugs. Spirometry and the measurement of pulse rate and blood pressure are advised in all patients receiving topical beta-blocker therapy.

“…47,49,57 And lastly, although systemic side effects of topical medications are uncommon, they can be very serious. 58,59 In addition to side effects and nonadherence, topical medical therapy is limited by the inherent difficulty of penetrating the corneal surface, the eye's natural barrier, to deliver medication in a consistent 24/7 manner. This difficulty may contribute to the fact that although all glaucoma medications have proven IOP-lowering efficacy, they are still not a sure-fire way to eliminate the risk of glaucoma progression.…”

“…Issues such as subclinical reversible airways disease, headache, dizziness and drowsiness may arise when utilising drugs like timolol or brimonidine. 214,215 In the potentially vulnerable geriatric population, these may lead to more severe consequences such as falls. In some cases, conducting preliminary cardiac (such as sphygmomanometry or electrocardiogram) and respiratory (such as spirometry) have been suggested to screen for subclinical disease.…”

“… 6 - 10 Noncompliance is especially significant when patients use more than one medication 11 and ultimately leads to suboptimal, inconsistent IOP control. By replacing or decreasing the need for topical medications, LTP can improve IOP control, reduce systemic side effects such as cardiovascular and respiratory adverse events and depression 12 13 and local side-effects, such as eyelid dermatitis, lacrimal system scarring, ocular discomfort upon instillation, tear film instability, subconjunctival fibrosis, conjunctival inflammation and epithelium changes and corneal surface and endothelial impairment. 14 …”
