Primary Carcinoid Tumor of the Ovary: MR Imaging Characteristics with Pathologic Correlation

Abstract: Ovarian carcinoid tumor is a rare neoplasm that may appear as a solid mass or often combined with teratomas or mucinous tumors. We report 2 cases associated with mucinous cystadenomas and describe their magnetic resonance imaging characteristics. On T 2 -weighted images, the tumors appeared as multilocular cystic masses with hypointense solid components as a result of abundantˆbrous stroma induced by serotonin. Demonstration of prominent hypervascularity of the tumors following contrast administration on dynam… Show more

“…Primary tumors are usually unilateral with a relatively benign course. They are usually solid tumors and often associated with mature cystic teratomas or mucinous tumors (87). As the venous drainage of the affected ovary bypasses the liver, carcinoid syndrome is noted in up to onethird of cases and is seen without liver metastasis (88).…”

“…At CT, they usually appear as solid enhancing masses often indistinguishable from other ovarian neoplasms (Fig 29). The solid components of the tumors have fibroma-like hypointensity on T1and T2-weighted images with diffusion restriction due to hypercellularity, whereas other benign fibrous T2-hypointense ovarian tumors usually show low signal intensity on diffusion-weighted images (87). On the other hand, mucinous carcinoid tumors can demonstrate high signal intensity on both T1-and T2-weighted images (90).…”

“…They are usually of primary gastrointestinal origin. 3,4 Primary ovarian carcinoids usually behave in a benign fashion but metastatic carcinoids are aggressive and associated with poor outcome. Hence distinction between pure primary ovarian carcinoid and metastatic carcinoid is crucial and it has alsoclinical and prognostic importance.…”

“…7 CDX2 expression by carcinoid tumors points to GI origin. 4 Midgut tumors and their metastatic tumors express high level of CDX2 compared to foregut and hindgut carcinoids. [8][9][10] CDX2 positivity is seen in tumors arising in small intestine(95%), appendix (92%) and duodenum (80%) but rare in tumors of colorectum (12%) and stomach (0%).…”

“…They are usually unilateral and mostly seen in the perimenopausal and postmenopausal women. Generally, they occur in association with mature cystic teratomas or mucinous tumors [3]. There are four histological types of primary ovarian carcinoid tumors; insular, trabecular, strumal and mucinous.…”
