Primary cutaneous follicle center lymphoma (PCFCL) is a cutaneous B-cell non-Hodgkin lymphoma (NHL), primarily seen in adults and only very rarely reported in children. Here, we describe an adolescent male with PCFCL of the scalp, treated with systemic rituximab monotherapy with a complete response.A 15-year-old male, with no significant medical history, presented with multiple erythematous lesions on his scalp. He first noted a nodule on his scalp approximately 9 months prior to presentation. This continued to grow larger and then spread with multiple other nodules appearing in the area, which prompted evaluation. He had no systemic symptoms such as fatigue, weight loss, fevers, or night sweats, and no other rashes or skin lesions. He had no significant travel history or insect bites. Physical exam was unremarkable, with the exception of a patch of scattered large papules in a 9 × 9 cm area with thinned hair overlying on his scalp (Figure 1).
