Primary health care tasks in implementing the main operations of public health

Background. Patient's satisfaction with health care facilities and services is one of the indicators to assess quality health care. Objectives. This study was intended to assess the level of satisfaction with delivery care and its associated factors among the mothers who delivered their child at health facilities in the urban areas of Bangladesh. Material and methods. This was a cross-sectional study conducted among women aged 15-49 years having at least one child aged two years living in an Urban Primary Health Care Project area. A total of 3949 women were interviewed. A two-stage cluster sampling technique was adopted to select the participants. Multinomial regression analysis was carried out to identify the potential predictors of level of satisfaction with delivery care. Results. The analysis revealed that the women were least satisfied with accessibility and quality of services provided during delivery care. They were satisfied with availability, support services and interpersonal communication of the health facility. Multinomial regression analysis revealed that the satisfaction level among women who received post-delivery counseling from the non-poor catchment area was 6.98 times higher, compared to 5.88 times who received such counseling from the poor catchment area. However, women from poor families appeared to be highly satisfied with delivery care in the poor catchment area (p < 0.001). Conclusions.Although the study revealed that the women were satisfied overall with the delivery of care, there is still much work needed to be done in the area of providers' work quality, interpersonal communication such as behavioral issues, and accessibility to the health services.

Ambushes related to collecting patients’ consent for medical procedures by family doctors

Performing a treatment procedure without previously collecting valid consent is a crime, resulting in criminal prosecution as severe as up to 2 years in prison. Collecting a patient's consent to be subjected to medical service can be divided into four main phases: checking if consent shall be collected under current circumstances; determining from whom it shall be collected; passing information to the patient or other person entitled to receive it; and actual collection of consent from the person entitled to express it. each of these can present as a potential ambush for physicians on multiple levels. Consent forms include: contextual, oral and written; and oral consent is quite often questioned by patients, as it is difficult to properly document the details of communication between the physician and patient during a visit. as consent for participation in a medical experiment always requires a written form preceded by passing on relevant information to the patient; it is worth remembering that, according to some interpretations, prescribing drugs off-label to a patent by a physician fits the broad definition of therapeutic experiment. in the context of a patient's consent to receive medical services, the rising need for documenting in medical files not only the patient's final decision, but quite often also many physiciandependent factors as subsequent steps that led the patient to reaching this decision, is noticeably rising. The role of primary care physicians in enabling validation of a patient’s ability to make legal statements and express a last will

Medical staff play an important and law-rooted role in providing valuable evidence in such cases. the forensic medical literature was searched for topics related to evaluations of last will and testament. Forensic opinions issued by the Forensic Medicine Department, wrocław Medical university, Poland, in years 2006-2016 were also examined for cases illustrating the role of physicians and medical files in the legal evaluation of contested wills. this allowed us to conclude that mental status alterations that prevent patients from making last wills in a legally valid way can result from psychiatric or somatic disorders that negatively influencing the level of consciousness. Medical files are a highly valuable and trustworthy source of evidence -especially in the parts authored by medical staff in frequent contact with patients, which includes primary and palliative care physicians or nurses. it is necessary that these files contain remarks on the patient's behavior and ability to function in a home environment, as well as information shared by the patient's relatives. it is crucial that any drugs prescribed and their doses are recorded, and that the actual drugs administered to the patient are also noted.
