Primary Pancreatic Lymphoma: An Uncommon Presentation in the Pancreatic Tail

Abstract: Primary pancreatic lymphoma is a rare form of pancreatic cancer that represents a diagnostic and therapeutic challenge due to its rarity and presentation mimicking pancreatic adenocarcinoma. Herein, we report a case of a 57-year-old Caucasian male who presented with left-sided chest pain, epigastric pain, and melena. Abdominal imaging was remarkable for a large, necrotic mass near the tail of the pancreas extending into the splenic hilum and left kidney. Biopsy of the mass confirmed lymphoma of B-cell origin. … Show more

“…Primary pancreatic lymphoma (PPL) is often, therefore, only diagnosed postoperatively [1][2][3] and not commonly diagnosed in the tail of the pancreas. 4 It is important to differentiate PPL from pancreatic adenocarcinoma because the treatment and prognosis vary significantly between these two entities.…”

“…Less than 2% of patients with NHL have involvement of the pancreas upon presentation. 2 Extranodal lymphoma is classified as secondary if there is involvement of lymph nodes or if there is more than one extra nodal site. Secondary involvement of the pancreas is more common than primary pancreatic lymphoma; however, the overall involvement of the pancreas is rare.…”

“…NHL usually originates in the lymph nodes, but can also present in primary extranodal sites, such as the stomach, skin, lungs, and rarely, the pancreas. 1,2 NHL can be a result of proliferation of B-cells, T-cells, or both, with B-cell lymphoma being more common than T-cell. 3 This case presents a rare example of B-cell NHL originating from the pancreas tail.…”

“…Radiologically, the lesions of PPL appear as focal and well-defined lesions or diffuse infiltration of pancreas ( 153 ), as summarized in Table 2 . The tumor is most commonly located in the head of the pancreas, though it can be found in other portions of the pancreas, such as the body and tail ( 149 , 154 ). PPL almost presents as bulky masses with a median size of 7.9 cm ( 155 ).…”
