Proatlantal intersegmental artery: a review of normal and pathological features

Abstract: The proatlantal intersegmental artery maintains the posterior circulation until the vertebral arteries are fully developed between the seventh and eighth gestational weeks. When this artery fails to obliterate, it becomes persistent one. The proatlantal intersegmental artery, most commonly, is an incidental finding or it may be of clinical significance in some patients.

“…Some vascular variations have been described coexisting with the PIA. Agenesis or hypoplasia of one VA is observed in 50% of patients with persistent PIA [10][11][12] . Absence of bilateral external carotid arteries with type I PIA has been reported by Basekim et al 1 .…”

“…It never runs into the cervical canal. It joins the vertebral artery at the V3 junction in the sub-occipital area before penetrating the foramen magnum 9,10 . Type II PIA (38%) originates from the external carotid shortly after its origin.…”

“…This plexus is initially supplied by three transient carotid-basilar anastomoses (trigeminal, optic, and hypoglossal arteries) and by the cervical intersegmental arteries between the hindbrain vascular plexus and anterior carotid circulation (Vasovic et al 2009). The carotid-basilar anastomoses involute concomitantly with the development of the PCoA between the 30th and 40th days of fetal life, while the first cervical intersegmental artery (the proatlantal intersegmental artery) participates in the formation of the VA.…”

“…Carotid-vertebrobasilar anastomoses (primitive trigeminal, otic, hypoglossal and proatlantal intersegmental arteries) are embryonic presegmental arteries supplying the primitive vertebrobasilar system from the primitive ICA [12][13][14][15]. The origin and termination of the right PCoA indicates the most cranial of the embryonic vessels connecting the anterior and posterior cerebral circulations.…”
