Profile Changes Following Maxillary Impaction and Autorotation of the Mandible

Abstract: This study showed that the change in the facial profile caused by autorotation of the mandible after Le Fort I osteotomy and maxillary impaction can be predicted in relation to the dimensions of maxillary impaction.

“…The maxillary impaction results in self-rotation of t he jaw, first described by Schendel a nd collaborators, in 1976; Bell and collaborators, in 1977 and Fish and collaborators, in 1978; such self-rotation is accompanied by skeletal and soft tissue changes observed in sagittal advance and vertical shortening of the lower face [5,12].…”

“…Maxillary impaction allows correction of the Gummy Smile, long face syndrome, specific types of open bite from a skeletal origin and labial sealing. 35 The Le Fort I osteotomy of the maxilla allows reduction of bone between the nasal floor and apices of maxillary teeth which permits superior repositioning (impaction) of the maxilla. 17 When having bimaxillary protrusion, the treatment option may be composed of first premolar extractions followed by the osteotomy through the extraction sites to mobilize the anterior segment of the maxilla.…”

“…In contrast, vertical pull chin cup appliances are effective in producing true anterior mandibular rotation and decreasing the anterior facial height 4442. For example, young patients treated with anterior-pull chin-cup therapy show twice as much forward movement of the chin, and almost three times as much true mandibular forward rotation, than matched controls 42.…”
