Profiling and hormonal therapy for acne in women

Abstract: Acne vulgaris is the most common condition treated by physicians worldwide. Though most acne patients remit spontaneously, for the ones that do not or are unresponsive to conventional therapy or have obvious cutaneous signs of hyperandrogenism, hormonal therapy is the next option in the therapeutic ladder. It is not strictly indicated for only those patients who have cutaneous or biochemical evidence of hyperandrogenism, but can be used even without any evidence of hyperandrogenism, for therapy-resistant acne.… Show more

“…Acne is not caused by a hormonal misfit, but hormones, primarily androgens and related receptors, can play a role in its pathophysiology and treatment . Before puberty, the adrenal glands produce increasing amounts of dehydroepiandrosterone sulphate (DHEAS), which can be metabolized into more potent androgens in the skin, driving enlargement of the sebaceous gland and increased sebum production; this is a prerequisite for acne in all populations.…”

While patients with hyperandrogenism benefit from an examination for underlying PCOS, virilism in PCOS is rare; patients presenting with virilism should be evaluated for endocrine malignancy and endocrine tumors (if viri ). Furthermore, there is a paucity of information regarding the prevalence of signs of clinical hyperandrogenism across the age spectrum in patients in PCOS.…”
