Prognostic factors for the clinical severity of keratoconjunctivitis sicca in patients with Sjögren's syndrome

Abstract: Serum anti-La/SSB antibody, serum anti-Ro/SSA antibody and tear IL-17 are likely to be strongly involved in the clinical severity of KCS in patients with primary SS.

“…One small study found excess pulmonary disease among patients with rheumatoid factor (70), while IgA rheumatoid factor was associated with renal disease in another cohort (71). The presence of rheumatoid factor was a marker of more severe exocrine gland manifestations (keratoconjunctivitis sicca) among 121 Sjögren’s patients followed at least one year (72). Thus, rheumatoid factor is a prognostic finding in Sjögren’s but may not be useful for clinical diagnosis or research classification purposes because this antibody is found commonly in other diseases.…”

“…High titers of anti-Ro and anti-La antibodies also have been shown to be associated with a greater incidence of extraglandular manifestations, especially purpura, leukopenia, and lymphopenia [23, 24]. …”

“…12,14 Because ocular surface epithelial damage could affect visual disturbance and indicates the presence of ocular surface in ammation, the OSS parameter is important for determining treatment for DED patients with pSS. 13,16,31 Previous studies have reported that an association between dry eye and systemic parameters, or extraocular variables. 11,16,32−35 They used the symptoms of ocular dryness, tear osmolarity, or severe DED severity, as indicators of DED.…”

“…Similar to our study, few studies used the OSS as the main outcome variable, but only reported correlations with laboratory ndings, such as positive serologic ndings. 15,16 Therefore, we focused on the OSS as the main outcome and analyzed its independent association with systemic parameters, laboratory ndings, oral parameters, and other ocular surface parameters after adjusting other variables.…”
