Prognostic significance of microscopic tumor extension in local recurrence of myxofibrosarcoma and undifferentiated pleomorphic sarcoma

Abstract: Myxofibrosarcoma (MFS) and undifferentiated pleomorphic sarcoma (UPS) frequently display infiltrative growth into the adjacent normal soft tissue. In this study, we aimed to determine whether the microscopic extension into surrounding normal tissue can influence the local recurrence of MFS and UPS. A total of 42 cases (22 MFS and 20 UPS) were examined. The length of the microscopic extension was measured from the edge of the main tumor mass to the end of infiltration. The length of 5.5 mm was selected as the o… Show more

“…The literature describes that failure of the primary surgical resection to achieve adequate negative margins can be a predictor of poorer prognosis [ 6 , 8 , 27 ]. The infiltrative nature of MFS as seen in histological observations and MRI findings [ 1 , 6 , 7 , 28 ] demonstrates the difficulties of obtaining adequate negative margins. Given the importance of accurate MFS diagnosis and surgical resection margins, an intraoperative assessment might be a necessary step.…”

“…Since this kind of tumor is apt to recurrence and metastasis, margin-negative surgical resection is the most effective therapy. Insufficient tumor-free margins are associated with a high risk of local recurrence and, hence, a poor prognosis [15,16]. The role of radiation therapy and chemotherapy on the treatment of primary MFS remains uncertain.…”

“…Our primary study outcomes were local recurrence rates, stratified by histologically defined surgical margins. The surgical margin was measured on histologic slides in millimeters from the resection surface to the nearest viable tumor [11,28]. The histologic status of the surgical margin was defined as microscopically positive (0 mm), negative (# 1 mm), and negative (> 1 mm).…”
