Prognostic Usefulness of Sentinel Lymph Node Biopsy for Patients Who Have Clinically Node Negative, Localized, Primary Invasive Cutaneous Melanoma

Abstract: To assess the prognostic value of sentinel lymph node biopsy status for patients with localized, clinically node negative, primary invasive cutaneous melanoma. Design: Predictive value of positive or negative sentinel lymph node biopsy (SLNB) results for melanomarelated death, using raw numbers from informative publications.

“…Despite the small number of patients in our cohort, our results confirm previous studies on SLN analysis in melanoma [3,4,8-11], in terms of SLN identification rate (97%), percentage of SLN positive patients (22%) and percentage of additional positive CLND (25%). We also observed a significant association between positive SLN and primary tumor thickness and microscopic ulceration.…”

“…In our study, the number of false-negative (10/153 = 6.5%) was similar (3-8%) to other studies [11,16,17]. However, there is ongoing debate on how to correctly calculate the false-negative rate.…”

“…In 2011, a Bayesian analysis reaffirmed the lack of prognostic value of SNB status in thick melanoma. 18 In contrast, multiple studies have demonstrated that the status of the sentinel lymph node is predictive of survival with node-negative patients having a 5-year survival of 56-71% versus 26-48% with node-positive disease. [30][31][32][33][34] In a recent metaanalysis by Rondelli et al, 35 and colleagues SNB status was predictive of survival with a 71% 5-year survival if node(s) was negative for disease and 39% if positive.…”

“…Several studies and a Bayesian analysis reported no prognostic value to SNB for thick melanoma. [16][17][18] In contrast, multiple other studies have demonstrated a predictive value for SNB in thick melanoma. [19][20][21][22][23][24] The therapeutic value of SNB for thick melanoma is also unclear.…”

“…The results presented by Vermeeren et al 13 suggest that the only relevant prognostic factor regarding patients with thick melanoma is SNB and this procedure can be used to stratify patients according to the risk of recurrence. Conversely, Rhodes 14 and Oliveira Filho et al, 15 stated that SNB status does not contribute with relevant prognostic information in patients with thick melanoma and, therefore, SNB should not be recommended due to the lack of prognostic information.…”
