Prognostic value of single nodal zone metastasis in non-small-cell lung cancer☆

Abstract: Patients with single nodal zone metastasis showed favourable outcomes compared with the multiple zone metastasis group. Even though two or more nodal stations were involved, the outcome was favourable if the nodal stations involved were confined to a single nodal zone. In conclusion, patients with single nodal zone metastasis can benefit from surgical resection.

“…This finding is in agreement with certain conclusions drawn from the surgical indications. For example, N2 cases with single positive zone might still be viable candidates for subsequent surgical resection [23]; however, induction chemotherapy should be strongly advised for cases of multiple stations (multiple nodal chain) involvement [24]. Moreover, to attain a more precise prediction of survival, comprehensive nodal information of all stations has been suggested for all N2 cases.…”

“…Further analysis with a larger population will be needed for a reliable conclusion on the long-term survival between these two groups. Kim et al [19] analyzed the survival data of 217 patients with ipsilateral mediastinal metastasis of their 1,186 patients with NSCLC, who underwent curative surgical resection. They reported that patients with single nodal zone metastasis showed favorable outcomes compared with that of the group with multiple nodal zone metastases [19].…”

Takamochi et al [20] reported that there were statistically significant differences between a single station N2 and a multiple station N2, but there was no statistical significance between a single zone N2 and a multiple zone N2.…”
