ObjectivesThe mechanism of pain sensation in patients with ulcer-like functional dyspepsia is unclear and there are no guidelines for its treatment. Whilst much research and many pharmacological treatments have provided symptomatic relief of ulcer-like functional dyspepsia through control of acid secretion, the role of acid in ulcer-like functional dyspepsia is unknown. We carried out this study to investigate the role of hypersensitivity to acid in the sensation of pain in ulcer-like functional dyspepsia by direct administration of acid into stomach.MethodsTwelve patients with ulcer-like functional dyspepsia(5 male, 7 female, mean age 41.0 years old, 7 with mild chronic superficial gastritis, 2 with metaplastic gastritis and 3 with normal endoscopic findings) and seven healthy control subjects(3 male, 4 female, mean age 36.0 years old, 7 with normal endoscopic findings) participated in the study. After overnight fasting, a nasogastric sump tube was inserted and its tip was positioned in the antrum under fluoroscopic guidance. Normal saline or 0.1N hydrochloric acid was administrated in random and single blind cross over fashion. The test was considered positive only if similar pain developed-similar to that experienced by the patient under nonstudy condition- during the administration.ResultsFive patients(41.7%) developed pain with 0.1N hydrochloric acid administration, but none with normal saline(p<0.05). None of the healthy controls developed pain upon administration of 0.1N hydrochloric acid or normal saline.ConclusionWe suggest that acid hypersensitivity plays a role in the sensation of pain in ulcer-like functional dyspepsia.

“…The present study revealed no differences between the four groups (MD, UD, control and PU groups) in patient profiles or histological findings of the stomach and gastric juice pH; and the fact that there was no difference between the functional dyspepsia and control groups in endoscopic findings agreed with previous reports. [26][27][28] Somatostatin reportedly releases into the interstitial space of the stomach and is thought to act on the target cells, for example gastrin containing G cells, in a paracrine fashion and then enter the gastric lumen. Circulating hormone levels are not directly relevant if the hormonal action is essentially para~rine.~' More than 90% of the SS immunoreactivity in the human gut is confined to the mucosal layer, where it is localized in D cells.30 Therefore, ir-SS concentrations of the mucosa and gastric juice were measured to ascertain the local changes associated with the symptom(s).…”

Immunoreactive-somatostatin (ir-SS) concentrations of the gastric mucosa and mood state in patients with functional dyspepsia were examined. The subjects were 12 patients with upper abdominal discomfort, nausea and/or vomiting (motility disorder group) and 14 patients complaining of upper abdominal pain (ulcer-like disorder group) for more than a month without any organic upper-gastrointestinal tract disease proven by endoscopy. These patients were compared with either an age- and sex-matched group of asymptomatic outpatients without any organic disease (control group: n = 26) or to a group of patients with peptic ulcer (n = 19). Somatostatin concentrations of the stomach were measured by radio-immunoassay, and the mood state of each subject was assessed by Manifest Anxiety Scale (MAS) and Self-rating Depression Scale test. Immunoreactive-somatostatin concentrations of the gastric mucosa were significantly higher in the ulcer-like disorder group than in the peptic ulcer, motility disorder or control group, and gastric juice levels were higher in the ulcer-like disorder group. The psychometric tests showed that the motility disorder group was more depressive than the ulcer-like disorder group, but there were no differences between the motility disorder, ulcer-like disorder and peptic ulcer group in MAS scores or environmental factors. These results indicate that there may be two different subgroups in functional dyspepsia influenced by both ir-SS concentration of the stomach and/or mood state.
