Programmatic Challenges in Obtaining and Confirming the Pneumococcal Vaccination Status of Cochlear Implant Recipients

Abstract: Ensuring optimal vaccination of all CI recipients against high-risk pathogens is a significantly challenging task. Maximizing vaccination rates in CI users will require an ongoing, active effort of information dissemination, documentation of compliance, and well-designed behavioral systems to streamline the pragmatic challenges in vaccination delivery.

“…Data presented in our study confirm that a sizeable percentage of implanted patients (71.8%) have not been vaccinated against S. pneumoniae , even though it is suggested by government agencies, otolaryngological societies, implant manufacturers, and cochlear implantation centers [ 25 ]. The rate of vaccination in the study group is significantly lower compared to data from USA clinics [ 24 , 25 ]. Although no patients were identified with post-implantation bacterial meningitis in the group, there continued to be new cases of meningitis in the general population.…”

“…There are a very limited number of papers reporting on vaccination against S. pneumoniae coverage in cochlear implant patients. Data from USA cochlear implant centers show that vaccination rates range from 49% to 99% [ 24 , 25 ].…”

“…12 However, it is consistent with medically at-risk paediatric pneumococcal vaccine coverage reported at 3-9% nationally, 13 and other international adult and paediatric CI cohorts in Poland (28%) 14 and America (20%). 15 Locally, educational letters and immunisation plans for children scheduled for or who had received CI were effective in improving immunisation rates (Tay SM, unpublished data), suggesting improved parent and provider awareness were key factors influencing optimal infection prevention.…”

“…Implementation of the recommendation of a supplemental dose of PCV-13 in patients previously vaccinated with PCV-7 required cochlear implant centers to contact patients and either refer them to primary care for immunization. Complete vaccination was only documented in 49% of adults and 58% of children with prior implants (17). Carpenter and colleagues launched a communication campaign to document vaccination status of their cochlear implant patients.…”

“…Few have reported on their attempts to achieve vaccination compliance. The efforts of Carpenter et al (17) published in 2010 are laudable for their successes; nevertheless, the authors stressed it was ''an immense challenge.'' These challenges led the authors to conclude that most patientsVeven after documenting their willingness to complyVwill not reliably obtain vaccinations on their own (17). They noted in 2007 that despite standard notices and verbal recommendations issued at office visits, only 20% to 30% of their cochlear implant patients were immunized at the time of surgery.…”
