“…With regard to the etiology of backwash ileitis and pouchitis, some reports have suggested that the inflammation is induced by ileocecal valve insufficiency and physical stimuli, including intestinal bacteria and bile acid, and a correlation between the etiology of these lesions and the mechanisms of occurrence of UC has also been strongly suspected. 25,26 In a recent study, lesions of the small intestine were recognized in all 15 patients with refractory pouchitis examined by capsule endoscopy. 27 Moreover, inflammation proximal to the pouch in the neo-terminal ileum, so called pre-pouch ileitis, has been observed.…”

“…We have reported the specific and significant proliferation of immature plasma cells in the inflamed colonic [11] and pouch [12] mucosa of patients with UC compared with healthy controls and patients with CD. Although these immature plasma cells may have important roles in abnormal humoral immune responses, including autoantibody production, the mechanisms of migration of immature plasma cells to inflamed sites of UC have not been clarified.…”

“…For example, proliferation of immature plasma cells was increased in pouchitis [52][53][54][55]72]. Pro-inflammatory cytokines, such as TNF-a, are released mostly in the inflamed mucosa by macrophages and monocytes, leading to tissue injury, and are considered to be involved in the mechanism of pouchitis [54].…”
